Schoolcraft Memorial Hospital Physical Therapist in Manistique, Michigan

POSITION SUMMARY : Provides a variety of medical services to help individuals who have been injured or physically affected by illness to recover or improve function. The physical therapist must be able to evaluate a patient’s condition and devise a customized physical rehabilitation and treatment plan and modify as needed. Primarily works in primary department listed above but agrees to cover in other areas of SMH as needed/requested. This position is responsible for any patient requiring therapy throughout the organization (Inpatient, Outpatient, Swing-bed, Homecare and Hospice).

QUALIFICATIONS:

  • Graduate of an approved school of Physical Therapy.

  • Licensed or eligible in the state of Michigan.

  • Able to tolerate physical requirements to perform patient treatment.

  • Ability to communicate in both written and verbal form.

  • Must have own transportation.

  • BLS Certified

    DUTIES & RESPONSIBILITIES:

  • Direct patient care, involving both evaluation and treatment of referred patients.

  • Comprehensive and Timely documentation of evaluation(s), discharge, and daily progress notes.

  • Communication with physicians and others team members involved in the patient’s care as needed or required.

  • Practice therapy consistent with the APTA code of ethics and the Michigan State practice act.

  • Participates in the CQI program and assists with quality improvement projects as needed.

  • Assist with Quality Improvement projects as requested.

  • Attend departmental meeting, hospital inservices, and continuing education as directed.

  • Must maintain patient confidentiality and abide by hospital wide policies of conduct.

  • Supervision of physical therapist assistants.

  • Provide supervision/evaluation of students as assigned by the CCCE.

  • Commits to the Behavioral Standards of SMH.

  • Demonstrate appropriateness in meeting objectives in age-specifics.

  • Performs other duties as assigned by the department manager.
