UW HOSPITAL & CLINICS RN Navigator, Pulmonary - 34245 in MADISON, Wisconsin
JOB REQUIREMENTS: RN Navigator, Pulmonary - 34245 100% FTE, Monday - Friday, between the hours of 8:00am - 5:00pm. Hours may vary based on the operational needs of the department. We are seeking an RN Navigator to: * Work in collaboration with numerous multidisciplinary teams to help navigate patients and families through Pulmonary care. * Be a conduit of information between inpatient and outpatient services and pre- and post-surgical care for patients and families. * Participate in continuous quality improvement initiatives. * Work closely with Pulmonary providers, carry out coordinated plan of care for patients, and be highly organized and great at multi-tasking.
