The Interventional Clinical Program Coordinator functions as an integral member of multi-disciplinary teams for coordination and preparation of patients for various interventional procedures and follow-up of patients. Responsibilities include coordinating patient care between dialysis centers, referring providers, hospital and clinics to provide continuity across settings. The Interventional Clinical Program Coordinator assists in monitoring performance measures and adherence to established standards of practice and evidence-based guidelines and facilitating collaboration and communication between departments and services for efficient patient transition.

    Requirements:

  • Graduate of a Bachelor’s of Science in Nursing (BSN).

  • 2 years of procedural nursing experience.

  • RN licensure in the state of Wisconsin.

  • CPR certification or eligible to complete within 3 months of hire.

  • Driver’s license (for transportation to Outreach events).

  • Understands the nursing process and can apply to patient care, optimizing patient outcomes.

  • Understands and supervises delegated tasks to unlicensed personnel.

  • Enjoys working with variety of patient ages and conditions/diagnosis.

  • Excellent organizational, internal and external service excellence skills.

  • Exceptional verbal and written communication skills.

  • Ability to present oneself as a professional.

  • Ability to work well in a fast-paced, ever changing environment with multiple priorities.

  • Ability to work well independently and within teams.

  • Ability to prioritize work in an efficient manner.

  • Ability to recognize and respond to urgent needs in a timely manner.

  • Use of usual and customary equipment used to perform essential functions of the position

    Preferred:

  • Experience in committee membership and participation.

  • EPIC experience.

  • Quality/Process Improvement Training.

  • Understands concepts chronic disease management (i.e. end stage renal disease).
