The Hospitalist Nurse (HN) under the supervision of Director of Medical/Surgical Nursing and Hospitalist Services, functions in a supportive role to the Hospitalist and health care team as well as patients and their families in order to facilitate the delivery of quality, timely, and appropriate care and discharge. The HN is responsible for the assimilation of information related to assessment, planning, implementation, and evaluation of patients’ care needs in order to maximize the efficient utilization of resources. The HN collaborates with the Hospitalists and the hospital interdisciplinary care team to coordinate and manage the process of patient care. The HN collaborates to identify patients with complex social/medical needs and/or who are at high risk for hospital re-admission and assists with the development of treatment plans aimed at reducing premature re-admissions. The HN provides a primary link for post discharge needs, assisting with post discharge physician appointments, and acting as a liaison between the inpatient hospitalist program and post discharge services. The HN actively participates in providing outstanding customer service and accepts responsibility for maintaining relationships that are equally respectful to all.

The Hospitalist Nurse position requires strong leadership, communication and organization skills; tact and sensitivity to patient/family needs and issues; commitment to professional standards of conduct, practice, and ethics; and the ability to work well with other health care professionals in a multi-disciplinary model. The HN requires the ability to meet deadlines in an efficient manner in the midst of changing priorities. Minimum Qualifications:

  • BSN from an accredited School of Nursing. Will consider other nursing degrees on an individual basis.

  • 2 years of hospital experience

  • Positive skills in communication and experience which indicate successful ability to assume responsibilities within a team context. Must be willing to be a productive team member and interested in contributing to the continuous improvement of quality patient care.

  • Licensure as a Registered Nurse in the State of Wisconsin.

  • Knowledge of inpatient and outpatient workflow and disease management.

  • Knowledge of care coordination, including understanding disease processes and their impact on patient/family activities of daily living and lifestyle, and community resources for health care and equipment.

  • Flexible in responding to the changing needs of the organization; demonstrates critical thinking and problem-solving skills.

  • Proficient computer skills. Will need to work with EPIC.
