SNF Traige LPN (Fully Remote)
Part-Time: 2 position available immediately
Updated Available Schedule:
1) Friday-Sunday 6am-2pm CST
2) Friday-Sunday 10am-6pm CST
The Triage LPN - SNF provides On-Call and Triage for the assigned population in accordance with triage protocols. The tele-triage LPN assess health status, identifies problems, and needs of person(s) served in collaboration with other LPNs, facilities, and providers. You will handle incoming phone calls and perform outbound contact based on individual need.
- Triage all symptom-based calls and give recommendations according to approved triage protocol
- Triage patients by assessing severity of symptoms and collaborate with facility/providers to determine appropriate care
- Document in real time all care related interventions and recommendations to ensure local operations are notified
- Communicate with facilities and providers via EMR system and phone as necessary
- Ensure compliance with state/local requirements through proper documentation
- Participate in performance improvement activities and maintain ongoing clinical knowledge
- Notify facility and providers of all encounters regarding patients
- Actively participate in applicable meetings as requested
- Two years of experience in a clinical healthcare setting
- High school diploma/ GED required; Associate's Degree or some college preferred
- Licensed Practical Nurse (LPN) with an active license; compact license preferred.
- Experience with electronic medical record software and proficient use of the Microsoft Office suite
- Ability to multitask, prioritize, and manage multiple projects simultaneously
- Excellent written and verbal communication skills and ability to work independently or in a team setting
