Job Description:Smiling Angels Home Health Agency is seeking a compassionate and dedicated RegisteredNurse (RN) to join our team in providing high-quality home health care to our patients in theLouisville, KY area. The ideal candidate will be responsible for assessing patient health needs,developing and implementing care plans, and providing nursing care in accordance withphysician orders and the patient’s individualized plan of care.Responsibilities:1. Conduct comprehensive assessments of patients' health status and needs in their homes.2. Develop and implement individualized care plans in collaboration with the patient, family, andhealthcare team.3. Provide skilled nursing care, including wound care, medication management, IV therapy, andother treatments as prescribed.4. Monitor and evaluate patient progress and update care plans as needed.5. Educate patients and their families on disease management, medication regimens, andself-care techniques.6. Coordinate care with other healthcare providers and community resources to ensurecontinuity of care.7. Document all patient care activities accurately and in a timely manner.8. Adhere to all regulatory requirements and agency policies and procedures.
