UCLA Health RN-Assistant Director, Comprehensive Care Coordination in Los Angeles, California



this role, you will work to develop and implement innovative approaches

to comprehensive care coordination and chronic disease management across the

UCLA Ambulatory Health System. This role will provide leadership and direction

to promote teamwork, cooperation and development of Ambulatory Care

Coordination programs and systems. This role will develop strategy and

initiatives for the department, implements strategic initiatives with

key players in ambulatory operations, Office Of Population Health, ISS, Human

Resources, and outside health Systems with the goal to improve care

coordination across the spectrum. Motivates team members and establishes

departmental priorities and goals, plans, prioritize and uses effective time

management. Is a strategic leader in identifying areas to improve and expand

Care Coordination and develop chronic disease management programs for primary

and specialty care. The AD Has fiscal responsibility for developing, monitoring

and managing departmental finances. This role

will be a key driverin creating and leading a team focusedon an integrated and accountable approach to caremanagement, care transitions, care redesign, use of appropriate sitesof care, team building, and coordination with other existingcomponents of the UCLA Health System and community partners, clinical operations, protocol

and policy review, and communication of operations to the health system.


Minimum 10 years of advanced nursing or administrative Nursing experience. Master's Degree required, preferably Masters In Nursing Degree. BSN is required.

Skill at clearly communicating medical information to a broad variety of people, both in person and on the telephone

Advanced Clinical knowledge of medical diagnosis, procedures, protocols, treatments, and terminology

Experience with various computer software and hardware including word-processing, knowledge of Microsoft word, excel, outlook, CareConnect, internet, cisco, finesse.

Strong interpersonal skills: ability to effectively work with a diverse team of individuals in group setting.

3-5 years of supervisory experience required

Experience working with Ambulatory Clinic

Excellent communication skills; verbal, written and interpersonal.

Superior analytical and problem solving skills.

Strong leadership skills and independent decision making ability.

Experience using an Electronic Medical Record required. EPJC use preferred.

Ability to multitask and set priorities independently

Prior experience in training or education development preferred

Skill in recognizing an emergency or high priority situation

Skill at clearly communicating medical information to a broad variety of people, both in person and on the telephone.

Ability to exercise diplomacy and flexibility in interacting with doctors, nurses, administrators, and other personnel
