Bon Secours Mercy Health RN (Registered Nurse) Case Manager Inpatient Rehab Mercy Health Lorain in Lorain, Ohio

The Rehab Admissions Coordinator is responsible for the coordination of all Acute Inpatient Rehabilitation Unit admissions and referrals.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES:

A licensed RN in the State of Ohio, BSN preferred. Three (3) to five (5) years of experience in nursing. Two (2) to three (3) years’ experience in Acute Inpatient Rehabilitation Facility or Inpatient Rehabilitation nursing required. Knowledge of reimbursement for inpatient rehabilitation facility and experience working with third party payors is required. Must be able to pass a FIM assessment with written materials Requires a thorough understanding of medical terminology and procedures, good communication and interpersonal skills, and good clinical assessment skills Requires good problem solving skills and good judgement and initiative. Knowledge of current CMS admissions rules, regulations, and policies. Knowledge of CARF and Joint Commission standards and regulations for accreditation. Working knowledge of a wide variety of insurance carriers (i.e. Medicare, Medicaid, Dual Insurances, Commercial Insurances, HMOs, PPOs, etc.) Ability to thoroughly review patient charts to ascertain appropriateness for consideration into the Acute Inpatient Rehabilitation Unit setting. Excellent written and oral communication skills. Strong organizational and leadership skills. Must possess self-direction, motivation, and time management skills. Must be able to develop and maintain effective working relationships with people at varying ages, education, social, economic, and ethnic backgrounds. Basic computer skills – word processing and spreadsheet applications. Flexibility to adapt and respond to rapidly changing environment. Possess valid state of Ohio driver’s license or arrangements for own transportation; able to provide necessary documentation regarding insurance as requested

WORKING CONDITIONS:

Normal office environment with little exposure to noise, dust, temperature and the like.

The Rehab Admission Coordinator is responsible for on-site evaluation and completion of preadmission assessment forms, beginning orientation of patient and families, communication with discharge planners, and physicians. Collaborates with Rehab Admissions Coordinator and assists in case management for those on the unit. Maintains contact with internal and external referral sources to assure good communication and maximum referral opportunities
