Trinity Health Part-Time (12 Hours) Hospice Physician in Livonia, Michigan

Position Purpose:

In Accordance with the Trinity Health at Home (THAH) Mission, the physician is qualified to provide inpatient medical care to hospice patients who require hospitalization for symptom management, respite care, or medical management of the patient within a community setting. The physician is committed to making hospice services available to all those who would benefit and participates in on-going educational programs to increase their expertise in caring for hospice patients. In addition, the hospice physician will work effectively and communicate well with the members of the hospice team and fulfill all other requirements of the position.

Supervision Received: Reports to Hospice medical director. Works in collaboration with the executive director and Chief Medical Officer.

What You Will Do:

  • Display expertise in providing symptom management for hospice patients.

  • Completes FTF (face to face visits) with patients for purposes of admitting and recertifying patients, management of symptoms, and/or General inpatient visits.

  • Abides by all documentation requirements necessary for specific visit type.

  • All documentation must be completed at the time visit is performed.

  • Attend educational in-services offered by the hospice and possess an ongoing commitment to enhancing their medical expertise in caring for hospice patients.

  • Possess excellent communication skills with an understanding of working with patients with a terminal illness and their families.

  • Work closely with the medical director, the team physicians, and the team leaders to provide continuity of care consistent with the hospice philosophy and the patient's wishes.

  • Work collaboratively with all members of the interdisciplinary hospice team.

  • Provide 24/7 coverage, 365 days a year for any agreed upon on-call responsibilities and arranges coverage when unavailable. This information must be provided to the hospice Medical Director & hospice supervisor (designee) at least two weeks ahead of any scheduled absences.

  • Will assist in developing opportunities to conduct in-service presentations to the physician medical staff, the nursing staff, the social work and discharge planners and other hospital employee groups as needed.

  • Willing to assist with providing education to the interdisciplinary team as needed.

  • Complete all written documentation within 48 hours of visits, per our protocol.

Minimum Qualifications:

  • A physician (Medicine doctor, M.D. or Doctor of Osteopathy, D.O) licensed to practice medicine in state(s) practicing.

  • Ability to operate a motor vehicle required; must possess a valid driver’s license and have access to a functional automobile during work hours.

  • Is licensed to provide medical care and is a medical staff member in good standing with admitting privileges at least one hospital.

  • Complete the Trinity Health at home for hospice physicians within six months of their effective employment date.
