Job Description

  • Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values.

  • Ensures Patient Safety at all times

  • Utilizes core components according to AACVPR Guidelines for Cardiovascular and Pulmonary Rehabilitation/Secondary Prevention Program to maintain AACVPR National Certification and to provide quality rehabilitation that includes Evaluation, Interventions, and Expected Outcomes related to behavior modification and risk factor reduction associated with but not limited to:

  • Patient Assessment

  • Nutritional Counseling

  • Weight Management

  • Blood Pressure Management

  • Lipid Management

  • Diabetes Management

  • Tobacco Cessation

  • Psychosocial Management

  • Physical Activity counseling

  • Exercise Training

  • Special populations

  • Performs and documents accurate patient assessments

  • Develops individualized treatment plan (ITP) with collaborative patient goals. Reassesses the patient and updates ITP every 30 days and prn

  • Documents the need for a skilled level of care and services

  • Conducts patient education and training sessions

  • Evaluates/communicates patient progress/problems

  • Participates in team conferences, staff meetings, and in-services as appropriate

  • Monitors, records, interprets, and reports patient outcomes

  • Develops a home program (FITT) to promote long term adherence to recommended lifestyle changes

  • Implements continuous quality improvement principles

  • Initiates department emergency procedures as necessary

  • Provides in services to other departments as needed

  • Recommends rehabilitation to physicians and potential patients

  • Serves as role model through his/her attitude, communication style, and professionalism

  • Communicates with the referring health care physician to ensure that the patient is seen sufficiently often to assess the course of treatment and the patient’s progress and when appropriate-to adjust the course of treatment

  • Performs as patient advocate with strong belief in and understand goals of rehabilitation

  • Treats not only the disease but the patients as human beings, with empathy, and with compassion at all times

  • Takes personal responsibility to elevate education to a high level of expertise in field of care.

    Required Skills


  • Telemetry interpretation and exercise knowledge

  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals

  • Ability to write routine reports and correspondence

  • Ability to speak effectively before groups of customers or employees of organization

  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals

  • Ability to compute rate, ration and percent and to draw and interpret graphs

  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.

  • Ability to deal with problems involving several concrete variables in standardized situations

  • Demonstrate the knowledge, skills and abilities to effectively communicate to adolescent, adult and geriatric populations as well as develop treatment plans for adult and geriatric population

  • Must possess a valid SC license-RN, BLS,ACLS

ACLS certification within 6 months after employment
