Eastern Colorado Services For The Develo Community Support Specialist in Limon, Colorado

Function or purpose of the position Provide support and assistance to individuals with developmental disabilities receiving services, to allow them to lead independent lives both at home and in the community.

Essential tasks and functions specific to the position Address the needs of individuals receiving services while supporting them to become more independent. Be familiar with and adhere to agency Policies & Procedures, State and Federal mandates. Be familiar with and implement all aspects of the person-centered plan. Possess knowledge of emergency procedures and be responsible for each person's safety and welfare. Assist in supporting the needs of the individual receiving services including medication administration, medical supports, first aid and transportation. Provide opportunities for meaningful community involvement. Assist in general care, upkeep and maintenance of facility and grounds while keeping supervisor informed of any maintenance needs. Work with Director to develop outreach efforts, create community connections and inclusive coalitions. Facilitate the skill acquisition and use of technology including alternative and augmentative communication devices, mobility aids, e-mail, Facebook, and Twitter. Complete and submit all required data and reports. Work in partnership with the NECALG Case Manager and make referrals as appropriate to facilitate DD eligibility determination, paid services and supports, or referrals for other community resources. Complete required paperwork, (i.e.: ISSP/BISSP's, assessments, incident reports, activity log, etc.) in a timely manner. Implement all provided training techniques as applicable. Attend staff meetings and other trainings as required by the Agency. Ensure compliance with State/Federal Rules & Regulations. Support the role of caregivers and family involvement in the giving of care, support persons served and encourage self-advocacy for those served. Participate in team meetings and make sure the person served is able to attend and participate. Act as a liaison between the community and persons served. Perform other duties as assigned.

Necessary knowledge, skills, and abilities

Advanced degree in the field of intellectual and developmental disabilities preferred. Must have High School Diploma or equivalent and at least 2 years' experience in working with intellectually and developmentally disabled adults. Must possess valid driver's license. Must pass criminal and motor vehicle background checks. Must possess the ability to communicate effectively and work productively in a collaborative environment. Must be at least 18 years of age. Must have proficient computer knowledge. Strong organizational skills required. Prior supervisory experience preferred.

Physical demands While performing the duties of this job, the employee is regularly required to sit, use hand to finger, handle, or feel objects, tools, or controls; reach with hands and arms; hear and have the capacity to communicate on a verbal basis. The employee must have adequate visual acuity with or without correction and is required to lift heavy weights up to 50 lbs. and help with transporting and assisting individuals in services, to include lifting them when they need help. Must travel to meetings, training, agency facilities and other job-related locations. The employee is occasionally required to stand, walk and stoop; kneel and lift when necessary.
