Type: Full-Time: 40hrs/week
Salary : $18.31-$18.91/hr DOE

Key Responsibilities:
Client Assistance: Assists patients in the clinic setting to provide support and guidance on various resources and benefits.
Program Communication: Explain the benefits of the Community Health program to clients/patients and how it can improve their wellbeing.
Health Management: Help patients identify socio-economic issues affecting their health and develop management plans.
Documentation: Maintain comprehensive electronic client files, documenting encounters, referrals, and outcomes.
Service Coordination: Facilitate communication and coordinate services between providers and clients to ensure effective care.
What We’re Looking For:
Proficient computer skills, including internet-based applications
Ability to thrive in a fast-paced environment while managing multiple priorities
Strong organizational skills and accountability for time and activities with remote supervision
A passion for serving others with professionalism and clear boundaries
Community involvement or membership in the community served
Requirements:
High School Diploma or GED
Driver's license
Additional Certifications/Trainings upon hire:
BLS Certificate
Community Health Worker Certificate within the first six months of employment
Certified Application's Counselor (CAC) within one year of employment
