Full Time, Days

Flexible Schedules Available – have the schedule that works best for you!

  • Three 12-hour shifts per week doing admissions

  • Four 8-hour shifts working Friday through Monday (work 32 hours and get paid for 40 hours)

  • Five 8-hour shifts on a rotating schedule

  • Four 10-hour shifts on a rotating schedule

  • Float nurse – willing to work in any program (Home Health, Hospice, and Maternal Child Health)

RN Case Managersat the VNH screen, evaluate, re-evaluate, and treat patients as well as coordinate and communicate treatment activities with our interdisciplinary team members and the patient.

The RN Case Manager:

  • Develops a care plan in collaboration with the patient's physician, the patient, and their family, or the primary caregivers upon admission. Periodically reviews the patient care plan and recertifies the patient care plan as appropriate.

  • Coordinate the patient's care plan through interdisciplinary communication and case conferences, as appropriate. Advise other disciplines of discharges in a timely manner.

  • Provide ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and/or progress toward desired goals. Communicate significant changes to the patient's physician and VNA colleagues.

  • Document evaluations, treatment goals, and objectives.

  • Teach the patient and caregiver self-care techniques as appropriate. Provide medication, diet, and other instructions as ordered by the physician and utilizes opportunities for health counseling with patients and caregivers.

Minimum Qualifications:

  • Must be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license.

  • Graduate of an accredited school of nursing.
