Dartmouth Health Health & Aging Resource Specialist in Lebanon, New Hampshire
Overview
Assists patients, families, community members, and health care providers with accessing resources, information, education, and other supportive services related to aging. Supports the efforts of the Geriatric Center of Excellence.
Responsibilities
Answers and triages inquiries received by telephone, mail, or in person.
Provides assistance in the identification and referral to resources for the health care team and patients/families. Meets with patient/family to assess the need for services. Consults, educates, and directs the health care team to clarify the need for services. Determines the availability of services required. Provides patient/family information concerning available resources. Assists patient/family in obtaining appropriate resources. Maintains expertise in resources for older adults and their families.
Provides information to patient/families to secure needed financial and health assistance.
Maintains tracking log of activities and Community Resource Directory.
Coordinates and implements Chronic Disease Self-Management evidence based curriculum.
Identifies and contributes to the development of systems and resources needed to meet the support needs of patients/families and staff/institution.
Manages, maintains, and updates consumer lending library resources and database information systems.
Assists in the coordination of workshops, support groups, educational sessions, and special events.
Assists in marketing health programs and the D-H Aging Resource Center.
Trains and oversees volunteers.
Performs other duties as required or assigned.
Qualifications
Bachelor's degree in Human Service or related field with 2 years of applicable experience in progressively more challenging roles or the equivalent in education and experience.
Master’s degree preferred.
Excellent interpersonal, communication, and organizational skills required.
