Family Partner - Lawrence Community Behavioral Health Center (CBHC)

Job Description:

  • Sign on bonus up to $3000 (pro-rated for part time)

  • Bilingual Candidates Helpful (Spanish/Portuguese)

  • Full Time, M-F, 11:30am-8:00pm

Family Partner's work collaboratively with CBHC Clinicians to respond to youth/families seeking behavioral health crisis services. The family partner provides on-going support to families/caregivers by assisting with navigation of the behavioral health system in person or via telehealth. The Family Partner can support brief interventions that address behavior and safety

Responsibilities:

  • Support families throughout the treatment and referral process, for the purpose of identifying, assessing, treating and stabilizing the situation. Support the parent/caregiver’s capacity to improve or resolve the youth’s emotional or behavioral needs and strengthen their capacity to parent.

  • Work one-on-one and maintain regular frequent contact with the parent(s)/caregiver(s) in order to provide education and support throughout the stabilization and referral process. May assist the parent(s)/ caregiver(s) in articulating the youth’s strengths, needs, and goals to external providers to enhance service delivery.

  • Educate parents/caregivers about how to effectively navigate the child-serving systems for themselves and about the existence of informal/community resources available to them; and facilitate the parent’s/caregiver’s access to these resources.

  • Complete timely and accurate documentation to reflect services provided.

  • Work with family towards meeting goal(s) established in the discharge plan.

  • Provides follow-up and support to families for up to 7 days as assigned.

  • Help families identify and secure resources in the community.

Qualifications:

  • Must have lived experienced as a parent or caregiver of a child with special needs who has experience navigating behavioral health and family serving systems.

  • High school diploma or GED and a minimum of two years of experience working with children/adolescents/transition age youth; and experience in navigating any of the child and family serving systems and teaching family members who are involved with the child and family serving systems.

  • Basic computer skills required. Knowledge of Excel, Intermediate Word, and of electronic medical records desired.

Schedule : Monday - Friday (11:30am-8pm)
