Army National Guard Units MEDICAL CASE MANAGER in Latham, New York

Summary THIS IS A NATIONAL GUARD TITLE 5 EXCEPTED SERVICE POSITION. This National Guard position is for a MEDICAL CASE MANAGER, Position Description Number T5885000 and is part of the MEDICAL COMMAND, National Guard. This vacancy does not meet the criteria for appointment of annuitants. Responsibilities As a Medical Case Manager, GS-0601-10, you will facilitate medical readiness and medical retention for National Guard military members. Requirements Conditions of Employment Qualifications SPECIALIZED EXPERIENCE: 1 year equivalent to at least next lower grade level. Experience interpreting and translating SM limitations from non-military providers. Experienced in draft language in web-based system and submit recommendations for SMs with health limitations to profiling military provider for consideration and signature. Must have experience with assisting in ongoing patient care coordination until planned outcomes are achieved. Experienced in using automated systems to document and track a variety of data related to case management and care. Experienced in integrating personal data files with other available automated systems to provide information required for briefings, presentations, reports. Experienced in maintaining local mobilization medical readiness information databases and spreadsheets to provide monthly status reports to units and OSS. Must have experience problem solving with the team, on an individual or system basis to correct problems, prevent negative outcomes and inefficiencies. Education Bachelor's or graduate/higher level degree: major study in an academic field related to the medical field, health sciences or allied sciences appropriate to the work of the position. You must provide a copy of Official Transcripts as documentation to support your educational claims. To receive credit for Education, you must provide documentation of proof that you meet the Education requirements for this position. Additional Information If you are a male applicant who was born after 12/31/1959 and are required to register under the Military Selective Service Act, the Defense Authorization Act of 1986 requires that you be registered or you are not eligible for appointment in this agency (Register | Selective Service System : Selective Service System (sss.gov)). This position is designated as Essential Personnel and may be subject to duty in preparation for, or in response to, a state emergency or disaster declaration. Head of Agency requests to exceed 14 days in a calendar year must be approved in advance by the National Guard, Director of Manpower and Personnel (NGB-J1). Superior Qualifications and Special Needs Pay-setting Authority may be considered for this position. TAG is final approval authority for incentive programs (applicable to newly appointed federal employees or those with a 90 day break in service).
