Adecco US, Inc. Case Manager RN in Las Vegas, Nevada

Adecco Healthcare & Life Sciences is hiring remote Case Manager RNs! This role is remote/work from home and all equipment is provided. You must reside within Nevada for consideration. Please read below and apply with an updated resume for consideration:

Type: Four-month contract, opportunity for extension or fulltime offer

Schedule: Monday through Friday, 8:30am to 5pm

Pay: $37.47 to $39.47 an hour

Responsibilities of the Case Manager RN:

· Assess and create a discharge plan to ensure patient receives quality medical care. Implement care strategies that are individualized for patients and directed toward the most appropriate and the least restrictive level of care.

· Utilize both company and community-based resources to establish a safe and effective case management plan for hospitalized members and effective discharge planning to prevent readmissions.

· Collaborate with patient, family, and health care providers to develop an individualized plan of care that encompasses both acute care episode and post hospital discharge plan.

· Communicate with all stakeholders the required health related information to ensure quality coordinated care and services are provided timely to all hospitalized members.

· Advocate for patients and families as needed to ensure the patient’s needs and choices are represented and supported by the health care team.

· Work performed and completed onsite at various acute and alternative level of care facilities within the area.

· Floating may be required covering cases in another facility telephonically.

· Depending on EMR access/availability may be required to cross covering in multiple facilities based on the daily average census and PTO coverage

· Attend all Medical Management clinical rounds.

Qualifications for the Case Manager RN:

• Associates degree in Nursing

• Must reside within Nevada

• Current, unrestricted RN License in NV

• Valid Nevada drivers license, and maintain personal auto insurance coverage

• 2+ years of varied clinical experience within a hospital, with an emphasis on case management or utilization management
