Summary

POSITION SUMMARY: Provides direct nursing care to individuals in their place of residence in accordance with a physician certified Plan of Care and agency policies and procedures under the supervision of a RN.

ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

  • Provide age-appropriate nursing care in accordance with physician’s orders, and VNA policies and procedures.

  • Provides formal and informal patient education based on age-appropriate educational goals mutually set by patient/caregiver and members of the health care team.

  • Notify registered nurse of changes in patient’s condition and care needs. Seek consultation from RN as indicated.

  • Prepare clinical documentation per VNA policies/procedures and regulatory standards.

  • Perform follow-up communication tasks relating to direct patient care.

  • Participate in multi-disciplinary team conferences.

  • Assist with agency or community relations projects as delegated by team manager.

  • Participate in mandatory in services and education activities as required.

  • Accountable for utilization and practice of systems thinking, teamwork, and customer service.

  • Adhere to all agency policies, procedures, guidelines and standards.

SECONDARY FUNCTIONS: The following duties are considered secondary to the primary duties listed above:

  • Other duties as assigned.

JOB REQUIREMENTS

MINIMUM REQUIRED QUALIFICATIONS:

  • Current LPN licensure in the Commonwealth of Pennsylvania

  • Use of personal vehicle, valid driver’s license, and car insurance.

  • Minimum of one year med-surg clinical experience required.

  • Current CPR certification.

PREFERRED QUALIFICATIONS:

  • Home health experience preferred.

The development of hypothesis and potential solutions to problems involves careful interpretation, analysis and diagnosis. The individual must be able to collect data, establish facts, and draw valid conclusions. The individual must be able to think creatively with a degree of inventiveness, experimentation and intuition. They must be able to deal with a variety of concrete and abstract variables.

Mathematics: The following levels of ability are necessary to understand mathematical concepts and apply them to problem-solving situations.

  • Ability to add and subtract two-digit numbers.

  • Ability to multiply and divide.

  • Ability to perform operations with units such as inch, foot, yard, ounce and pound (and/or their metric counterparts).

  • Ability to add, subtract, multiply, and divide all units of measure.

  • Ability to perform the four operations with like or common decimal fractions. Ability to calculate surfaces, volume, weights, and measures.

EQUIPMENT USAGE REQUIREMENTS : MACHINES, EQUIPMENT, TOOLS, SOFTWARE: To perform the essential duties of the position, use of the following machines, equipment, tools, and software is customarily required:

PHYSICAL REQUIREMENTS : Degree of physical exertion is moderate, exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.

BODY POSITION AND MOVEMENT: The following positions or movements are typical of the job:
