Job Summary:
Updating patient specialty care plans
Preparing patients and providers for upcoming visits
Coordinating care with community partners
Coordinating post-hospital plans of care
Collaborating with the Interdisciplinary Care Team (IDC)
Nurse Care Manager Knowledge/Skills/Abilities:
Strong computer skills
Ability to multi-task at a very high level
Strong problem-solving abilities
Excellent phone/customer service skills
Familiarity with EMR software
Strong Clinical Background
Ability and willingness to expand clinical knowledge
Required:
Current Registered Nurse (RN)
Minimum 3 years of clinical experience

The pay range for this role in the state of Colorado typically falls between $39.00 - $45.00 hourly with the potential for performance-based bonuses and other benefits.
