Job Summary: Are you passionate about supporting individuals with behavioral health needs within a challenging environment? Join our team at Preferred Family Healthcare as a Jail Navigator.
You'll play a crucial role in coordinating care and facilitating appropriate referrals between the jail, court, behavioral health provider teams, and community resources. As part of a person-centered integration team, youll work to ensure individuals in crisis receive the support and care they need both during and after their incarceration.

Key Responsibilities:
Support individuals with behavioral health needs in accessing necessary resources and services.
Assist individuals with completing paperwork for diversion opportunities and behavioral health services during their jail stay and upon release.
Participate in Crisis Intervention Teams (CIT) or other initiatives for collaboration between entities working with individuals with behavioral health needs.
Aid jails in navigating the inpatient hospitalization process for individuals at risk of serious harm due to mental health disorders.
Coordinate with Behavioral Health Courts or other specialty/diversion courts to assist individuals with behavioral health needs.
Provide consultation on behavioral health issues for jail personnel.
Assess individuals with behavioral health needs and support with de-escalation as appropriate.
Ensure coordination of care with all relevant parties upon release from jail, including jail, court, behavioral health providers, and community resources.
Collaborate with local behavioral health providers and community entities.
Participate in critical incident debriefs for jail team members.
Complete all mandatory documentation as required by the supervisor.
Act as a member of the multi-disciplinary team.
Perform other duties as assigned.
Education and/or Experience Qualifications:
Bachelors degree required; Masters degree preferred, in a related field.
Two years of direct service experience with the target population or related human service experience.
Valid drivers license required.
Additional Qualifications:
Ability to build and maintain collaborative relationships and work effectively within a multidisciplinary team.
Skilled in conflict resolution and de-escalation techniques.
Self-directed with the ability to work independently.
Flexible schedule or willingness to respond to crises as needed.
