Job Summary: Are you passionate about supporting individuals with behavioral health needs within a challenging environment? Join our team at Preferred Family Healthcare as a Jail Navigator.

You'll play a crucial role in coordinating care and facilitating appropriate referrals between the jail, court, behavioral health provider teams, and community resources. As part of a person-centered integration team, youll work to ensure individuals in crisis receive the support and care they need both during and after their incarceration.

Key Responsibilities:

  • Support individuals with behavioral health needs in accessing necessary resources and services.

  • Assist individuals with completing paperwork for diversion opportunities and behavioral health services during their jail stay and upon release.

  • Participate in Crisis Intervention Teams (CIT) or other initiatives for collaboration between entities working with individuals with behavioral health needs.

  • Aid jails in navigating the inpatient hospitalization process for individuals at risk of serious harm due to mental health disorders.

  • Coordinate with Behavioral Health Courts or other specialty/diversion courts to assist individuals with behavioral health needs.

  • Provide consultation on behavioral health issues for jail personnel.

  • Assess individuals with behavioral health needs and support with de-escalation as appropriate.

  • Ensure coordination of care with all relevant parties upon release from jail, including jail, court, behavioral health providers, and community resources.

  • Collaborate with local behavioral health providers and community entities.

  • Participate in critical incident debriefs for jail team members.

  • Complete all mandatory documentation as required by the supervisor.

  • Act as a member of the multi-disciplinary team.

  • Perform other duties as assigned.

Education and/or Experience Qualifications:

  • Bachelors degree required; Masters degree preferred, in a related field.

  • Two years of direct service experience with the target population or related human service experience.

  • Valid drivers license required.

Additional Qualifications:

  • Ability to build and maintain collaborative relationships and work effectively within a multidisciplinary team.

  • Skilled in conflict resolution and de-escalation techniques.

  • Self-directed with the ability to work independently.

  • Flexible schedule or willingness to respond to crises as needed.
