Key Responsibilities:

  • Adherence to Code of Conduct: Provide all services and functions within Preferred Family Healthcare’s Code of Conduct, adhering to all company policies for employment.

  • Health Service Systems Understanding: Demonstrate an understanding of health service systems and assist program participants in identifying, securing, and sustaining resources needed to reduce crisis and/or increase stability.

  • Care Coordination: Coordinate care and collaborate with clinicians to develop individualized plans based on the strengths, needs, abilities, and preferences of the person served.

  • Community Resource Consultation: Consult and cooperate with community resources to facilitate linkage, referral, crisis management, and advocacy.

  • Family Involvement: Coordinate family involvement in the service delivery process to ensure holistic support for the client/patient.

  • Financial Navigation Assistance: Help clients/patients navigate financial entitlements systems to obtain adequate income and access to healthcare coverage.

  • Skill Development: Assist clients/patients in learning skills for successful personal health management and self-advocacy.

  • Mental Illness Understanding: Demonstrate an understanding of the overall nature, course, and treatment of mental illness.

  • Service Evaluation: Evaluate services provided to ensure necessary improvements are made to increase outcomes.

  • Continuity of Contact: Provide continuity of contact for the client/patient throughout the entire service process, ensuring consistent support.

  • Resource Coordination: Coordinate access to housing, social services, substance abuse treatment, and other community resources, and consult with these resources to help clients/patients achieve their goals.

  • Client/Patient Rights: Understand and respond to client/patient rights and advocate on their behalf, ensuring their needs and preferences are respected.

  • Community Linkage: Serve as a conduit and link between clients/patients and community services and resources, facilitating seamless transitions and support.

  • Additional Duties: Perform all other duties and responsibilities as assigned, demonstrating flexibility and a commitment to comprehensive client/patient care.

Education and/or Experience Qualifications:

  • Bachelor’s degree in social service, education, behavioral health, or other health-related fields from an accredited college OR experience providing case management in a healthcare field equivalent to a Bachelor's degree.

  • Documented experience in the area of case management services is preferred.

Additional Qualifications:

  • Knowledge of case management and rehabilitation methods, principles, and techniques, as they relate to mental and behavioral health.

  • Knowledge of the problems encountered by persons with mental illness and how to access various community resources.

  • Knowledge of methods and modalities, as well as legal requirements and court procedures related to treating mental illnesses.

  • Some knowledge of supervisory and training techniques and practices used in a mental/behavioral health setting.

  • Capable of communicating ideas clearly and effectively, both verbally and in writing to produce clear and concise reports.
