Summary:

  • The Penn Medicine at Home (PMAH) Home Health Aide is responsible for implementing a Plan of Care, assisting the patient/family with performing personal care tasks, and assisting with the activities of daily living. The individual is responsible for performing home health or hospice visits throughout a designated service area. A Home Health Aide is responsible for completing timely and accurate submission of patient documentation.

Responsibilities:

Patient Care:

  • Help patients with bathing, brushing teeth, skin and hair care.

  • Prepare and assist with light meals.

  • Help patients get dressed and use bedpans, commodes, and urinals.

  • Empty and record the amount from Foley catheters when needed.

Exercise and Transfers:

  • Support patients with their home exercise routines.

  • Assist with moving patients from bed to chair and turning them for comfort.

Patient Environment:

  • Perform household tasks and other essential services as outlined in the care plan.

  • Help with pre-poured medications that patients usually take themselves.

  • Provide temporary relief to the main caregiver when assigned.

Documentation:

  • Record patient care accurately and on time.

  • Complete required hours and report any schedule changes.

  • Sign and verify all activity sheets and ensure timely submission of reports for government reimbursement.

Communication:

  • Report any unusual changes in patients’ condition, safety issues, refusal of care, or family dissatisfaction to the primary nurse or supervisor.

  • Facilitate communication between patients' families, the RN Case Manager, and Scheduling Coordinator.

Plan of Care:

  • Follow the care plan regarding visit days and hours.

  • Provide care as outlined unless instructed otherwise by the Case Manager.

  • Review the care plan every two weeks before providing care.

Employee Travel:

  • Use your own transportation to visit patients and meet state driving requirements.

Education:

  • Attend annual training and complete at least 12 hours of continuing education each year.

  • Follow PMAH procedures and meet all required competencies for Home Health Aides.

Credentials:

  • Car Insurance (Required)

  • Certified Nurse Aid (Required)

  • Certification as a Nursing Assistant (in PA) or Home Care Aide (in NJ) depending on geographic location and scope of responsibilities or student nursing assistant in lieu of certification, matriculated in an accredited BSN nursing program having minimally completed the nursing foundations/fundamentals clinical course with a 3.0 GPA overall and in the nursing major.

  • Driver's License (Required)

Education or Equivalent Experience:

  • H.S. Diploma/GED (Required)

  • Equivalent Experience: 1+ years One (1) year recent experience as a Nursing Assistant or Home Health Aide Or 0-1 years Six (6) months experience as a Nursing Assistant or Home Health Aide commencing within two (2) months after completion of an approved Nurse Aide or Home Care Aide Training Program

  • Education Specialization: Graduate of a certified nursing assistant program-in lieu of certification, must be a nursing student matriculated in an accredited BSN nursing program having minimally completed the nursing foundations/fundamentals clinical course with a 3.0 GPA overall and in the nursing major and continue to maintain a 3.0 GPA in all nursing major subjects. In order for nursing students to maintain their position as a HHA, they must continue in a matriculated BSN program and present evidence with an official transcript documenting a 3.0 GPA overall and in the nursing major each semester to their supervisor/Regulatory Compliance. Nursing Students are required to submit grade reports/transcripts to their supervisor/Regulatory Compliance within 60 days of their course completion. Nursing students employed as HHAs, must immediately (within 24 hours) and in writing report to their supervisor and Regulatory Compliance if they have not sustained the academic GPA requirement or discontinued/withdrawn from their matriculated BSN program. Failure to meet and maintain nursing student requirements in lieu of certification will result in termination of your nursing assistant position.
