KENOSHA HUMAN DEVELOPMENT SERVICES CSP Registered Nurse in KENOSHA, Wisconsin

JOB REQUIREMENTS: 1. Medication management under the direction of the psychiatrist/physician\'s assistant; a. In collaboration with other program nurses and supervisors, develop and maintain medication management policies and procedures and training b. Evaluate and chart medication effectiveness, complications and side effects, and arrange for required lab work c. Organize and manage the system of medication delivery/administration in concert with case managers/service facilitators d. Manage and coordinate medications and medical supplies delivered to the agency 2. Medication administration of medications (oral and injection) in office or community as necessary 3. Case coordination a. Coordinate with case managers/service facilitators and administer health assessments of program participants\' physical health b. Act as primary support in cases requiring medical expertise -- attend medical appointments and conduct home visits with case managers as requested c. Consult with on-site and outside psychiatrists as well as primary care physicians to maintain coordination of treatment as needed d. Attend psychiatrist and or APNP case review meetings e. Make appropriate referrals to community physicians and medical facilities f. Review plan progress as it relates to medical, psychological and pharmacological concerns and make necessary adjustments to medication schedule with case managers/service facilitators g. Review and coordinate client signature on informed medication consent forms annually and as new medication is prescribed 4. Documentation a. Document participant contacts, medication administration and collateral contacts in Client Tracking System (CTS) andCCS system in accordance with Note Policies and Procedures b. Maintain daily medication administration charts to ensure accuracy of medication administration, office pick up and home delivery c. Document medication cycle fills to ensure accurate tracking of medication deliveries sent and delivery received by client d. Facilitate the transmission of medication prescriptions and refills to pharmacies as appropriate 5. Other duties a. Act as resource, and provide consultation, to any and all teams and to program participants b. Participate in team meetings c. Assist in training of program and agency staff as needed d. Act as advocate in the community for program participants e. Coordinate and administer physical health assessments for other KHDS program participants and employees at KHDS ***** OTHER EXPERIENCE AND QUALIFICATIONS: QUALIFICATIONS: Minimum of an Associates Degree from an accredited college in Nursing required. Bachelor\'s Degree preferred. 1. Ability to effectively communicate, both written and verbally, in the English language 2. Must have reliable transportation and valid driver\'s license with good driving record 3. Must carry own auto insurance with at least 100,000/300,000/100,000 coverage 4. Ability to maintain records and document case notes daily 5. Ability to give and accept feedback as a part of a team 6. Ability to traverse within the workspace and community frequently 7. Be willing to learn and work with the agency\'s treatment philosophies 8. Bilingual in Spanish a plus 9.
