KENOSHA HUMAN DEVELOPMENT SERVICES Crisis Linkage and Follow-Up Counselor in KENOSHA, Wisconsin
JOB REQUIREMENTS: RESPONSIBILITIES 1. Maintain knowledge of available community resources for consumer/family referral 2. Develop and maintain relationships with community resources a. Identify point of contacts with the community b. Provide linkage and follow up for crisis consumers/families c. Provide ongoing support to clients as referred by a supervisor from the debriefing team until other providers in thecommunity are available to help the person/family maintain stable functioning d. Assisting the person to engage in new services or current services; coordinate information and service delivery for the person e. Contacting the person\'s ongoing mental health providers or case manager, if any, to coordinate information and servicesrelated to the person\'s care and support f. Confer with other persons providing support for the person to determine if the response and follow-up are meeting theclient\'s needs g. Developing a new crisis plan or revise an existing plan to better meet the person\'s needs based on what has been learnedduring the mental health crisis 3. Provide inpatient hospitalization discharge planning for all voluntary and voluntary county funded individuals 4. Promptly complete all required documentation into CTS in compliance with DHS 34 statute 5. Refer and complete appropriate documentation for services as appropriate a. Assist in determining suitable placement for voluntary clients b. Refer/admit to KARE Center or Kauffman House for crisis stabilization c. May provide facilitating services (such as transportation) as needed 6. Perform individual employee requirements for continuing education and clinical supervision as required for agency statecertification 7. Perform other work as required by the Crisis Program Manager ***** OTHER EXPERIENCE AND QUALIFICATIONS: QUALIFICATIONS: Bachelor\'s degree in Human Services or related field required. 1. Ability to effectively communicate, both written and verbally, in the English language 2. Must have reliable transportation and valid driver\'s license with good driving record 3. Must carry own auto insurance with at least 100,000/300,000/100,000 coverage 4. Ability to maintain records and document case notes daily 5. Ability to give and accept feedback as a part of a team 6. Ability to traverse within the workspace and community frequently 7. Be willing to learn and work with the treatment philosophies 8.
