Job Purpose: The Caregiver I provides personal cares for clients needing assistance in the home to remain independent. This position is supervised by the assigned Community Liaison.
Essential Job Functions:
The Caregiver I shall be assigned by the Community Liaison and/or RN Supervisor to individual clients to perform specific tasks for each client per the client's Plan of Care. The individual must be trained to perform assigned cares and this training will be assured and approved by the Community Liaison and/or RN Supervisor. The individual is limited to performing only those tasks and services as assigned for each client and for which he or she has been specifically trained.
Understands and performs basic personal care and/or supportive home care tasks such as housekeeping bathing and mouth care, grooming, dressing, toileting, eating assistance, and skin care and reports findings to the community liaison and/or registered nurse.
Assures confidentiality of all patient/family information.
Completes daily documentation in EHR to include all pertinent information specific to individual client care facilitation and program involvement.
May attend interdisciplinary plan of care conferences for assigned individuals.
Practice and promote safety in the workplace; reports all accidents according to agency procedures.

Experience working with individuals with disabilities in a day service, residential, or community setting is preferred.
Ability to pass Direct Care Competency Program and obtain a Direct Care Competency Certificate provided by the agency.
Maintains confidentiality of all agency and client related information and follows HIPAA regulations.
Maintains proper attendance and punctuality in accordance with agency attendance policy.
Adhere to ethical business practices by striving to perform in a manner that conforms to the highest standards of ethical behavior, integrity, and honesty.
Work Environment and Physical Requirements
The environment is community-based. Work occurs in the client's home and errands in the community as assigned. The work environment and physical demands described here are representative of those that an employee may experience or must do to successfully perform the essential functions of the job. Physical demands vary depending on client.
Ability to lift 50-100 pounds and sometimes in excess of 100 pounds, with the use and or assistance of a mechanical or Hoyer lift.
Frequent stooping, kneeling, reaching and the ability to climb stairs.
