Logan Health Medication Safety Program Manager in Kalispell, Montana

Job Description Summary:

Logan Health is seeking a dynamic and self-directed Medication Safety Program Manager to join our Quality Safety team!

This critical role is responsible for overseeing medication quality and safety programs, identifying opportunities to improve medication-use systems, and developing error prevention strategies aimed at reducing patient safety risks. You will review event reports, track medication errors, conduct investigations, and address safety issues. This position leads the Diversion Committee, and serves on the USP 800 committee. The role also involves close collaboration with healthcare providers to monitor and measure medication use systems and implement error prevention strategies.

As part of this role, you will work closely with pharmacy directors, managers, and leadership teams on process improvement initiatives to enhance safety practices throughout the hospital. The ideal candidate must be a licensed pharmacist with a passion for safety, a collaborative spirit, and experience in hospital pharmacy or related fields. Completion of a fellowship in medication safety is preferred but not required.

Qualifications:

  • PharmD from an accredited college of pharmacy required.

  • Current Montana State Board of Pharmacy, Registered Pharmacist License required .

  • Advanced study in pharmacy or health care related field preferred.

  • Possess knowledge of medication-use safe practices; ISMP initiatives; National Patient Safety Goals; and process-improvement methodologies such as Lean and Six Sigma required.

  • Possess knowledge of pharmacy information systems and automated dispensing technology required.

  • Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.

  • Commitment to working in a team environment and maintaining confidentiality as needed.

  • Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.

  • Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.

  • Possess and maintain computer skills to include working knowledge of Microsoft Office Suite and ability to learn other software as needed.

Job Specific Duties:

  • Serves as the authoritative resource on medication safety and as Chair of Medication Safety Committees as applicable to assigned area(s).

  • Participates in the development, implementation and integration of patient-focused medication quality and safety initiatives in collaboration with leaders and key constituents as applicable to assigned area(s).

  • Participates in, and/or leads, multidisciplinary teams regarding errors, events, reactions, policy review, new product review, and patient safety as applicable to assigned area(s).

  • Collects, reviews, and analyzes established departmental and system-wide Medication Quality and Safety programs. Identifies improvements; and develops and leads implementation of high-leverage error-reduction strategies.

  • Ensures compliance of state and federal medication safety regulatory and legal requirements as applicable to assigned area(s).

  • Investigates medication safety events, issues, and/or error reports. Utilizes data driven performance improvement methodologies and tools to develop recommendations for action.

  • Encourages system-wide medication error reporting. Fosters a culture of safety through "lessons learned" education and communication across the system. Supports principles of high-reliability organizations (HRO) within medication use processes and workflows.

  • Provides medication safety education and presentations on safe medication use as applicable to assigned area(s).

  • Oversees medication safety issues per department protocol.

  • Maintains knowledge of trends and developments in the patient safety field and collaborates with external medication safety organizations. Shares locally identified and reported medication quality and safety issues with national organizations and manufacturers as appropriate.

  • Provides guidance and coordination for medication related information systems and technologies. Provides operational, clinical, and safety recommendations related to installation, maintenance and enhancements of Pharmacy related systems and technologies. Ensures information system and technology processes support established clinical standards of care, meets patient care quality standards, compliance with various regulatory agencies, and integrates with other departments per department protocol.

  • Participates in the development, implementation, and evaluation of pharmacy information systems and technology to consistently meet customer's needs.

  • Maintains knowledge of leading-edge developments, best practices, and evolving information technology systems and solutions

Schedule:

Logan Health operates 24 hours per day, seven days per week. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
