UPMC LPN (Chemical Dependency Unit) in Jamestown, New York
Make a meaningful impact in the lives of individuals on their journey to recovery at UPMC! As an LPN in our Chemical Dependency Unit, you will play a vital role in our healthcare team, utilizing your nursing expertise to support patients facing addiction challenges.
Responsibilities:
Collaborate as a member of the healthcare team, exercising sound nursing judgment and adhering to UPMC LPN Practice Guidelines and State Practice Act.
Participate in the planning, implementation, and evaluation of patient care, utilizing focused assessments and communicating effectively with the healthcare team.
Formulate individualized care plans and daily goals, involving patients as partners in their care and ensuring a holistic approach.
Document patient conditions comprehensively to enhance communication between caregivers and improve patient safety and satisfaction.
Build caring, compassionate relationships with patients, families, and colleagues to create a supportive healing environment.
Provide detailed education and support to patients and families for effective transitions throughout the care process.
Foster the development of new LPNs and patient care support staff while actively seeking feedback for continuous professional improvement.
Identify opportunities for clinical quality and workplace improvements, collaborating with the healthcare team to achieve desired patient outcomes.
Demonstrate a commitment to a balanced work-life approach, promoting safe work practices and cultural understanding among peers.
Graduate from a State Board of Nursing-approved Licensed Practical Nursing Program.
Prior experience with mental health or chemical addiction population
Ability to thrive in a fast-paced environment, managing multiple priorities and specialized equipment.
Physical stamina for frequent walking, standing, lifting, and patient positioning; mobility and visual manual dexterity required.
Proficient in using electronic health records.
Licensure and Certifications:
Current LPN state licensure or multistate license as per the compact agreement.
Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification required.
Comprehensive Crisis Management (CCM) training within 60 days of hire; renew annually.
New York State Fingerprint Check; provisional work for 30 days may be allowed.
