Lutheran LPN Charge Nurse- Hultquist Place 2nd Shift in Jamestown, New York

Hultquist Place, Lutheran's Assisted Living Program is currently seeking Full-time LPN on 2nd Shift (2pm-10pm) to join our talented team of nursing professionals. This LPN will serve as the charge nurse for the 2nd Shift.

Salary Range: $21.00-$28.68 per hour.

Purpose:

To provide care to residents in accordance with facility policies, procedures and regulations under the supervision of the RN Case Manager and scope of practice for license. Also supervises and directs day-to-day work of other LPNs, HHAs, and PCAs.

Qualifications:

Graduation from an accredited school of professional nursing, current NYS License, current CPR certification, and basic working knowledge of computers and Microsoft Office

Job Duties:

  • Demonstrates an interest and competence in geriatric nursing

  • Provide care to residents following facility policies and procedures.

  • Processes MD orders and other resident appointment paperwork.

  • Assists with medications according to facility policies and procedures.

  • Observes 6 rights of medication administration when assisting with medications

  • Attends bi-annual medication review meetings and other monthly medication meetings regularly

  • Acts as charge nurse when needed.

  • Re-orders medications for the residents.

  • Recognizes significant changes in the condition of residents and reports to RN Case Manager

  • Report residents’ needs, concerns, and accidents/incidents to RN Case Manager Director of Clinical Services or Administrator.

  • Contributes to the improvement of resident care.

  • Makes resident rounds, discussing, noting and solving any problems

  • Assumes responsibility for the care of each resident as outlined in the Care Plan.

  • Completes incident reports and contacts medical providers as needed.

  • Performs direct care, when needed, per the resident care plan.

  • Assists in maintaining a physical, social and psychological environment, which will be in the best interest of the residents

  • Maintains and facilitates good relationships with families, residents and visitors.

  • Ability to cooperate with other nursing staff and interdisciplinary teams.

  • Takes shift report from staff of preceding shift, writes shift report and gives report to staff of next shift.

  • Completes all documentation of the medical record timely, completely and accurately.

  • Follows all regulatory standards and facility procedures for documentation.

  • Maintains accurate and complete records.

  • Participates in performance improvement for all processes.

  • Maintains an active HCS account
