Job Summary and Qualifications

The Social Worker promotes patient-centered care by evaluating psychosocial needs, overseeing community service coordination for identified patient populations, and providing support and crisis intervention where indicated. The Social Worker creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. The Social Worker facilitates patient movement across the continuum of care and identifies, addresses, and tracks barriers to patient throughput.

What you will do in this role:

• You will perform a comprehensive assessment of psychosocial needs of assigned patients.

• You will assess patients discharge needs and facilitate the provision of services necessary to meet the identified needs.

• You will act as a liaison between the patient and community resources and post-acute care providers.

• You will develop an individual plan of care for recurring patients to include education related to accessing healthcare services at the appropriate level of care.

• You will provide education to the under-resourced patient/family of potential and available resources.

• You will evaluate suspected abuse and neglect referrals and make an official report.

• You will act as a liaison through effective and professional communications between and with physicians, patient/family, hospital staff and outside agencies

• You will document professional recommendations, care coordination interventions and case management activities.

Qualifications You Will Need:

• Master's degree in Social Work or Counseling from an accredited university.

• Current State Licensure for independent clinical practice (LCSW, LPC, or LMFT).

• Prior experience working with a psychiatric mental health population, preferably in an inpatient setting.

• Prefer experience with case management, discharge planning, group leadership skills, age specific growth and development, crisis and behavior management, family therapy theory and practice.

• Excellent communications, organizational and documentation skills required. Must be able to work independently and with a variety of health care disciplines. Able to remove barriers for patient care and coordinate transition of patient into community care.
