Crisis Diversion Services On-Call Case ManagerPosition entails providing Crisis Diversion Case Management services (de-escalation, connecting patients with needed resources, coordinating appropriate treatment and assisting in dispatch when needed.) to agency patients.
general responsibilities: Must be able to develop a therapeutic rapport with diverse individuals of the targeted population. Must be able to assess and function in a strengths based model of care. Must be able to link patients with resources, coordinate appropriate treatment, and provide documentation for the clinical record. Assist with dispatch functions as needed.
essential job functions:
- Quantity: Achieves minimum of 600 hours per year (50 hours per month) direct patient contact. Direct contact includes both billable and non-billable services focused on achieving on CDS outcomes.
- Clinical: Maintain updated training in case management with an emphasis in crisis education. Assist patient in examining their strengths and accessing resources to cope with the current situation. Respond with face-to-face contact within one (1) hour for emergent situations, three (3) hours for urgent situations and twenty-four (24) hours for routine situations or as scheduled (i.e. welfare checks). Oversee Attendant Care workers after hours.
- Judgment: Assist QMHP in evaluating and determining the most helpful intervention strategy for a patient based on the patients input, history, and available resources. Clinical judgment is utilized in crisis or ongoing treatment based on agency policies and standard practice. Utilize independent decision making. Inform regular staff of crisis contacts with patients within one working day and be available 24 hours per shift.
- Coordination: Maintain effective working relationships with resources outside the agency. Assist staff in diversion process and following through. Active participation in meetings both within and outside the agency as need for patient care.
- Documentation: Documentation reflects progress toward diversion and treatment goals, and is accurate and timely.
