Southern Ute Tribe Community Health Representative in Ignacio, Colorado

POSITION: Community Health Representative.

DUTIES: The Community Health Representative is responsible for providing assistance and the coordination and development of activities for elderly and physically or mentally disabled Tribal members. Performs community health duties to include patient transport and monitoring, home visits, conducting classes, and providing community education on Tribal health issues. Home Care may include house cleaning, personal hygiene, preparing meals, and other health and wellness-related activities.

REQUIREMENTS: Have a High School Diploma or Equivalent. Have 6 months experience or training in a work setting in a medical field or working with and aiding the elderly and physically or mentally disabled. Have basic computer skills in software programs including Microsoft Word, Excel, and Outlook. Have experience providing personal care, i.e., toileting, bathing, grooming. Must or be able to obtain experience with medical equipment such as blood glucose monitor, blood pressure cuff/stethoscope, and pulse oximeter. Must have or be able to obtain Certification in First Aid and CPR within 3 months of employment. Be able to work flex hours and maintain an on-call schedule.

OTHER REQUIREMENTS: Hiring preference will be given to enrolled Native Americans. One year's experience in personal care, patient mobility and transfer, or the medical care. Experience giving public presentations and instructing individuals in a class setting. Ability to speak and write the Ute language. C.N.A. (Certified Nurse's Assistant) CHR Basic 100 Certification. Must have a valid Driver's License for state of residency and be insurable under the Tribal vehicle insurance policy. Must pass a thorough background check including Criminal History, fingerprint check, and pre-employment drug test.

WAGES: This position is a grade N9; pay range starts at $20.26/hour.

SHIFTS: Monday - Friday (8:00 am - 5:00 pm)

JOBS SITE: Ignacio, CO.
