Responsibilities
Work with Registered Nurse Care Manager to provide timely and efficient identification of patients who need discharge planning. Provide assessments of psychosocial impact of illness. Provide direct services including discharge planning for patients needing community resources prior to discharge from the acute care setting.
Comprehensively assesses a patient's goals as well as their biophysical, psychosocial, environmental, and discharge planning needs and financial status.
Works collaboratively with RN Care Manager to develop appropriate discharge plans and ensure indicated referrals are completed in a timely manner.
Procures services and serves as an advocate on behalf of patients and families for scarce resources.
Acts as a liaison to post hospital care providers and community health resources.
Qualifications
MINIMUM QUALIFICATIONS:
Education:
Licensed Certified Social Work
Licensed Master Social Work
Licensed Bachelor of Social Work with minimum of 5 years medical social work experience
Licensure:
- A current unencumbered Social Worker license to practice in the State of Arkansas is required.
Experience:
- Experience in a medical setting preferred.
Skills:
Must be able to communicate effectively, both written and oral.
Must possess organizational skills, able to prioritize work.
Must possess demonstrated training, knowledge, and skills necessary / appropriate to the neonate, infant, adolescent, adult, and/or geriatric age groups.
Pay Range
$26.38 - $38.26 /hour
