Project Director - Children to Adult Transition in Augusta, ME
The Department of Health and Human Services (DHHS) provides supportive, preventative, protective, public health and intervention services that safeguard the public health and assist our customers in meeting their needs.The Office of Child and Family Services (OCFS) within DHHS is dedicated to helping create a future where all Maine children and families are safe, stable, happy, and healthy. The Office of Aging and Disability Services (OADS) within DHHS is responsible for ensuring the safety, support and well-being of adults throughout Maine.
Overview
The Transition from Children’s to Adult Project Director will oversee the four Transition from Children’s to Adult Transition Liaison contract staffing positions (see the details in that job description) and develop and refine processes and procedures for four transition liaison positions which will be housed within OCFS and OADS. They will coordinate within OADS and OCFS program areas to assure quality and improvement activities are developed and conducted according to rule and policies with the two offices and the Office of MaineCare services as well as other DOE, DOL and DHHS staff to ensure alignment of transition activities. This position will be an important member of the team which is implementing a life-span waiver at OADS to help address many of the barriers between OCFS and OADS.
Section 9817 of the American Rescue Plan Act of 2021 (ARP) provides states with a temporary 10 percentage point increase to the federal medical assistance percentage (FMAP) for Medicaid HCBS expenditures. The temporary funding must be used by March 31, 2024 to improve or enhance Medicaid HCBS. HCBS provides critical supports and services to thousands of older adults and individuals with disabilities in Maine, enabling them to live in their homes and communities, rather than in nursing homes and other institutions.
Duties and Deliverables
The Transition from Children’s to Adult Project Director duties include:
Develop and refine processes and procedures for four transition liaison positions which will be housed within OCFS and OADS
Coordinate within OADS and OCFS program areas to assure quality and improvement activities are developed and conducted according to rule and policies with the two offices and the Office of MaineCareservices
Coordinate with other DOE, DOL and DHHS staff to assure transition activities are aligned across those departments
Ensure streamlined applications for MaineCareservices and waiver programs, for which individuals may be eligible
Develop processes which support increased quality oversight of transition process and services received during this time
Work with liaisons to develop processes with goal of reduced wait-times for services/case management between OCFS and OADS
Facilitate stakeholder meetings throughout the project with the goal of improved youth and family satisfaction
Work with liaisons to support connecting youth to long-term employment and housing opportunities to maximize independence
Develop policies and procedures with goal of ensuring access to supports in the least restrictive and costly setting
Work with stakeholders, other departments and transition liaisons to reduce potentially preventable hospitalizations and other service utilization that may result from gaps in care
This position will be an important member of the team which is implementing a life-span waiver at OADs to help address many of the barriers between OCFS and OADS
Be familiar with and know how to use Microsoft Office 365 programs, including SharePoint, Excel, Word, Outlook, PowerPoint and other electronic tools to accurately record and display data and information
Participate in initiatives as assigned to support the Department’s and Office’s goals and objectives
Qualifications
Experience and Education
Eight (8) years of education and/or professional experience in social services program development; and/or social services related program/policy/fiscal/budget administration/analysis work;
Other related areas that demonstrate the ability to think systemically, while also being specific in task related detail.
Preference will be given to applicants with a Master’s degree in social services or a related field, experience in conducting program compliance work, and experience with overseeing projects related to transition for youth with disabilities within state or federal systems, such as the Department of Education (DOE), Department of Labor (DOL), or Department of Health and Human Services (DHHS). Facilitation skills of meetings that include a various group of interested stakeholders will be required.
This is a full time 40-hours per week position
