Queen's Health System Patient Care Coordinator, Interventional Cardiology (Coronary/PCI Program) (Full-Time, Day, 40) in Honolulu, Hawaii
RESPONSIBILITIES
I. JOB SUMMARY/RESPONSIBILITIES:
• Collaborates with all members of the Queen’s Heart Institute (QHI) to coordinate the care of the patient across the continuum.
• Assists physicians and team with planning and coordination of the coronary program, data collection, coordination of procedures, quality improvement.
II. TYPICAL PHYSICAL DEMANDS:
• Essential: finger dexterity, seeing, hearing, speaking.
• Continuous: sitting, static gripping of an object for prolonged periods.
• Frequent: walking, repetitive arm/hand motions.
• Occasional: standing, stooping/bending, climbing stairs, walking on uneven ground, lifting usual weight of 25-50 pounds with assistance, reaching above, at and below shoulder level, frequent gripping of an object.
• Operates computer, calculator, telephone, fax and printer.
III. TYPICAL WORKING CONDITIONS:
• Not substantially subjected to adverse environmental conditions.
• Work environment is fast paced due to multiple demands.
• Requires ability to flex hours to meet department needs.
• Requires some outer island travel.
IV. MINIMUM QUALIFICATIONS:
A. EDUCATION/CERTIFICATION AND LICENSURE:
• Current Hawaii State License as a Registered Nurse.
• Bachelor’s Degree in Nursing.
• Current certification in Basic Life Support at the Healthcare Professional Level.
B. EXPERIENCE:
• Two (2) years’ experience in outpatient and/or acute care nursing preferably in a cardiac setting.
• Experience to demonstrate:
o Knowledge and proficiency to use computer applications including word processing, spreadsheets and presentation type software.
o Knowledge of electronic based medical records.
o Knowledge of performance improvement, databases/information systems preferred.
