Queen's Health System Clinical Pharmacist, Ambulatory - MSSP ACO (Full Time, 40, Day) in Honolulu, Hawaii

RESPONSIBILITIES

I. JOB SUMMARY/RESPONSIBILITIES:

• Implements, maintains and provides clinical pharmacist services including: clinical, educational and informational functions for newborn, infant, pediatric, adolescent, adult and geriatric patients of The Queen’s Development Corporation (QDC).

• Visits and provides clinical pharmacist services to patients in the community.

II. TYPICAL PHYSICAL DEMANDS:

• Essential: Standing, sitting, stooping, bending, walking, finger dexterity, seeing, hearing, speaking.

• Frequent: pushing/pulling usual weight of up to 40 pounds of force, lifting or carrying up to 30 pounds, reaching above, at and below shoulder level, frequent gripping of an object.

• Continuous: repetitive arm/hand motions.

• Occasional: kneeling, crawling, climbing stairs, walking on uneven ground, squatting, static gripping of an object for prolonged periods, pushing/pulling up to maximum 50 pounds.

• Operate pagers, computers, printers, typewriters, telephones, fax machines, and other office equipment.

III TYPICAL WORKING CONDITIONS:

• Not substantially subject to adverse environmental conditions.

• Work environment is fast paced, congested and may be stressful due to workload.

• Interacts with patients, nurses, physicians and the general public.

• Works on multiple tasks at the same time.

• May be exposed to communicable diseases.

• Must have access to motor vehicle.

IV. MINIMUM QUALIFICATIONS:

A. EDUCATION/CERTIFICATION AND LICENSURE:

• Current Hawaii State license as a Pharmacist.

• Doctor of Pharmacy degree.

• Valid U.S. driver's license required in accordance with Hawaii state law rules and regulations.

• Driver's abstract or evidence of good driving record may be required based on area of assignment.

• Post-graduate clinical residency accredited by the American Society of Health-System Pharmacists.

• Board certification preferred.

• Current certification as a Certified Diabetes Educator (CDE) preferred.

• Current certification in CPR at BLS/HCP level.

B. EXPERIENCE:

• Clinical ambulatory care pharmacist experience preferred.

• Experience to demonstrate ability to communicate professional expertise to all levels of the health care team.
