Queen's Health System Clinical Navigator - Neuroscience Institute (Full-Time, Day, 40) in Honolulu, Hawaii

RESPONSIBILITIES

I. JOB SUMMARY/RESPONSIBILITIES:

• Responsible for the coordination of patient care throughout the continuum, in collaboration with healthcare providers and multidisciplinary care team, to provide quality, comprehensive services for assigned area(s).

• Serves as an advocate, educator and clinical resource for area(s) assigned.

• Serves as a liaison throughout the facility and in the community regarding services provided for this unique patient population.

• Coordinates all aspects of the perspective treatment plan regimen related to assigned area(s).

II. TYPICAL PHYSICAL DEMANDS:

• Essential: finger dexterity, seeing, hearing, and speaking.

• Frequent: walking, standing, sitting.

• Occasional: lifting up to ½ pound, static gripping of an object for prolonged periods, stooping/bending, climbing stairs, walking on uneven ground, lifting and carrying usual weight of 1 pound up to 5 pounds, reaching above, at and below shoulder level, pushing and pulling up to 30 pounds.

• Infrequent: kneeling, pushing and pulling up to 35 pounds, horizontal, lifting/carrying up to 30 pounds, floor to waist up to 30 pounds, waist to shoulder up to 5 pounds, and bending.

• Operates computer, calculator, telephone, fax and printer.

• Operates various specialty equipment, i.e. machines, tools, etc. based on assignment.

III. TYPICAL WORKING CONDITIONS:

• Not substantially subjected to adverse environmental conditions.

• Exposure to bodily fluids and communicable diseases.

• Work environment is fast paced due to multiple demands.

• Must flex hours to meet department needs, as needed.

IV. MINIMUM QUALIFICATIONS:

A. EDUCATION/CERTIFICATION AND LICENSURE:

• Current Hawaii State license as a Registered Nurse.

• Bachelor’s degree in Nursing.

• Current certification in BLS/HCP.

B. EXPERIENCE:

• Two (2) years experience as a Registered Nurse in an acute care and/or outpatient setting.

• Experience to demonstrate the following:

o Ability/skill in patient teaching.

o Ability to use electronic based medical records and Microsoft Office applications.

• Experience with spreadsheet applications, clinical databases and performance improvement activities preferred.

Assignment to Acute Hemodialysis:

• One (1) year case management experience highly desired.

• Prior dialysis experience preferred.

Assignment to Orthopedics:

• Prior orthopedic experience preferred.

Assignment to Wound Care:

• Two (2) years experience as a Registered Nurse must be in a wound care setting.

• Master’s degree in Nursing preferred.

• Current certification in Wound and Ostomy preferred.
