Summary This position is located at Edward Hines Jr. VA Hospital a level 1a facility. This position falls under the Social Work service and is the Post-9/11 Military 2 VA (M2VA) Case Manager and the point of contact for COMPACT Act. This Social Worker will be instrumental in providing connection and referral to assist with proper evaluation for eligibility in other programs and is the facility subject matter expert on the COMPAT act. Salary Ranges: GS 9: $68,621 - $89,206 GS 11: $83,025 - $107,929 Responsibilities Incumbent is a professional Social Worker whose duties and responsibilities relate to the case management of Post 9/11 Service members and Veterans treated at the facility. The incumbent assists Post- 9/11 service members and/or Veterans in coping with acute illness, chronic illness, combat stress, the residuals of traumatic brain injury (TBI), community adjustment, addictions, and other health and mental health problems. The social work case manager addresses home care needs, homelessness, and transitions across levels and sites of care. Social Work case management practice, which includes psychosocial assessment, diagnosis, and treatment, is focused on helping service members and/or Veterans and their families maximize rehabilitation and treatment potential and achieve more adequate, satisfying, and productive emotional and social functioning. Must have a high level of skill and expertise to work with Post-9/11 service members and/or Veterans and families who are experiencing a wide range of complicated mental, emotional, behavioral, physical, psychosocial, and environmental problems Evaluates the need for mental health services and makes appropriate referrals for individual, group, marital, and family treatment services Must be sensitive to the ethnic and cultural diversity and age-specific challenges of the Post-9/11 population and adjusts intervention and treatment plans as appropriate. Serves as an advocate for Post-9/11 service members and/or Veterans and their families, helping them access needed services at the facility, at other VA facilities, and in the community. Travels as may be required, as part of providing social work case management services to Post-9/11 service members and/or Veterans and their families. Such travel requires the incumbent to function without immediate supervision or consultation Develop and monitor administrative and clinical plans for Veterans receiving COMPACT Act services Provides reports and metrics to facility, VISN and VACO as requested Captures and analyzes local data trends to inform program development and improvement Collaborate with local facility revenue and finance departments Advocate for Veteran, ensuring they do not receive bills for COMPACT related care Provide education and training on COMPACT Act to internal and external stakeholders as the facility subject matter expert Collaborate with interprofessional teams within the VA healthcare system and community to navigate complex care coordination SW Service is committed to developing and preserving a culture of diversity, inclusion and acceptance and we encourage all qualified candidates to apply. Basic Requirements: Citizenship: Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part). Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the 3 accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. GRADE DETERMINATIONS: GS-9: None beyond the basic requirements. GS-11: Licensure/Experience: Requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non VA experience) and licensure or certification in a state at the independent practice level. *Education substitution for experience- In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting Reference: For more information on this qualification standard, please visit https://www.va.gov/ohrm/QualificationStandards/. Physical Requirements: This work is performed in an office setting although some slight physical effort may be required to conduct trainings, give presentations or to observe staff and trainees in their work environment. This includes inpatient and clinical areas. This position will utilize a desk and computer and may need to navigate throughout the facility clinical areas.
