Responsibilities

Nursing Delegation:

  • Conduct 4 hour medication training to all newly hired direct care staff

  • Oversee medication processes within the programs offering guidance as needed

  • Documentation of all nurse delegation training’s/in-services, as required.

  • Ensure review of medical records for all new admits with-in 1 week of admission, or sooner, if appropriate.

  • Ensure conduction of resident specific trainings for residents in need of diabetic care, wound care, catheter care and other medical issues outside of basic direct care staff trainings, [i.e., Medication assistance, G-tube feedings, vital signs, blood glucose monitoring and appropriate documentation.

  • Have a thorough understanding of the nurse practice act and how it applies to nurse delegation.

Job Duties:

  • Ensure scheduling/conduction of resident labs as required.

  • Ensure tracking and review of lab reports and communicate to physician as needed.

  • Ensure conduction of assessment of residents that are more medically involved as medically indicated to include bowel and bladder assessment and appropriate documentation.

  • Ensure nursing and direct care staff responds to all emergency situations and completes all required documentation.

  • Ensures resident medical issues are addressed.

  • Ensures communication of pertinent information to the treatment team

  • Ensure identification and monitoring of residents that require regular/weekly weights, vital signs, hydration, skin checks and monitoring of blood pressure and other point of care testing.

  • Monitor medication management for residents according to MD orders, and assist as needed

  • Take verbal orders per nursing practice standards, offer guidance to obtain physician signatures for verbal orders and standing orders and ensure appropriate orders for discontinuation of medications, labs, and other clinical needs, per policy.

  • Ensure resident/family education and assist as needed.

  • Coordination of nursing issues with appropriate staff (i.e., clarification of MD orders, Medications, dressing changes, post admission instructions, etc.)

  • Ensure completion of skin checks, and other nursing and clinical procedures per SOP [Lippincott]

  • Ensure accuracy of MARs (as needed)

  • Ensure all transitional medications are reordered as needed.

  • Assist with staff training and delegate training as appropriate.

  • Ensure that appropriate medications and clinical information is sent with residents for appointments and outings.

  • Make appropriate recommendations to physician for medication changes, considering costs of certain medications on the resident and the company.

  • Attend resident transitional and supportive meetings as indicated per facility or delegate an appropriate representative to attend.

  • Participate in the referral calls and admits/discharges/transfer calls and other scheduled meetings as needed to ensure appropriate placement and facility readiness to accept and receive resident.

  • Participate in transitional site visits as needed.

  • Ensure regular [at least weekly] communication to the primary therapist, clinical manager, and case manager on any medical related goals for the residents’ treatment plans and monthly progress notes.

  • Conduct follow-up investigation on all Critical Incidents and Issues Reports regarding nursing, safety, and other residential clinical issues.

  • Participate in staffing’s/family conferences as needed.

  • Participate in in-service programs, case conferences, and clinical record reviews as needed.

  • Access continuing education opportunities appropriate to discipline in order to maintain clinical competency.

  • Adhere to facility policies and procedures and participate/lead facility quality improvement process to include performance improvement and safety program.

  • Ensure nursing staff and other direct care staff adheres to nurse report and communicate adherence needs with leadership if process is broken.

  • Act in compliance with Residential licensing, regulatory, accreditation and professional standards and guidelines.

  • Participate in On-call process as indicated.

  • Provide support and guidance to staff members to enable them to complete their resident care duties.

  • Coordinate and conduct annual performance reviews following appropriate guidelines for respective team members.

  • Provide input to Residential Manager regarding performance of care givers and direct care staff regarding medication or attention to clinical, safety and medical concerns for performance reviews and on an ongoing basis as needed.

  • Facilitate and ensure development, collection and reporting of nursing indicators, resident outcomes, infection controls indicators, medical and medication errors and team PI projects and reports.

  • Ensure systems are in place to maintain organization of nursing staff and maintain adequate and efficient nursing and/or direct care staff coverage throughout each day.

  • Complete staffing reports as required.

  • Interview prospective nursing employees as needed.

  • Provide guidance, direction, and supervision of employees within the nursing and direct care staff. Provide clinical support, oversight, and education, as needed.

  • Perform employee counseling and implement Professional Development Plans with employees as necessary. Collaborate with supervisor and Human Resources as necessary for assistance and in compliance with company’s corrective action policies.

  • Ensure ordering of resident and stock supplies [i.e., medical supplies, tube feeds] and that they are ordered and delivered timely, and proactively budgeted. Obtain MD orders as needed.

  • Ensures overall compliance to resident clinical and safety practices for nursing and direct care staff [i.e., infection prevention, medication management and skin integrity maintenance]

  • Ensure medication safety, storage including proper narcotic security, disposal, and administration.

Qualifications

  • Valid RN license in state(s) where services are provided

  • Current CPR licensure

  • Willing to pursue trainer certification of CPR and other related trainings and certifications

  • Working knowledge of applicable regulatory and funding source regulations

  • Minimum one year of supervisory experience in a healthcare setting

  • Must be able to relate positively with individuals and families

  • Demonstrate effective and professional interpersonal, verbal, and written communication skills

  • Possess the ability to handle concurrent tasks and responsibilities

  • Maintain a valid driver’s license, insurance, and an acceptable driving record per company policy and be able to travel within local area

  • Familiarity with accessible vans and other accessible equipment is preferred

Salary Range

USD $85,000.00 - $95,000.00 / Year
