Center for Rapid Recovery DSP-Medical Coordinator in Hempstead, New York

The incumbent must have a clean NYS driver license with the ability to drive a 7 to 14 passenger cargo van. The applicant shall be assigned to coordinate medical transportation and ensure the safety and protection of the individuals in the IRA or Day Habilitation Program, com Habilitation and Respite workers comply with all OPWDD Office of People with Developmental Disabilities regulatory requirements and safeguards and participate in ongoing training, administer, document and monitor the administration of medication. Report and document any incident involving the care or safety of the individuals. The incumbent is responsible for creating appropriate and required chart notes, rehabilitation plans, summaries and reports concerning the individuals participation and progress in the program. Various positions, Day Hab, IRA, Com Habilitation and Respite workers needed, Different shifts, Pt and FT available, 8 to 4, 4 to 12, 12 to 8 nights and weekends available.

Provide the appropriate level of supervision to ensure and protect everyones health and safety. Document daily process notes toward achieving Dayhab, ComHab, Respite, or IRA goals. Maintain anecdotal records and evaluations of all services Conduct fire and emergency evacuation drills Report and document all physical plant problems that affect the proper operation of the program Assist in the planning, preparation and implementation of activities Report to administration any physical, mental or sexual abuse to the resident including neglect and or maltreatment of any person Demonstrate appropriate modeling for the consumer Prevent injury to the consumer and others including self injury using approved techniques Other duties as assigned Must pass criminal history background check Bi-Lingual a plus Good communication and writing skills are required PPD required within one year or for evidence indicating the absence of TB
