Su Clinica Community Outreach Worker in Harlingen, Texas
Outreach staff will provide essential support to clinical services as needed during emergency situations. Supports include answering phone lines, directing calls, scheduling appointments. Contacting patients through telephone, home visits, hospital visits, clinic lobbies, patient mail out projects in order to schedule appointments, provide information on clinic services or health education information.
Educates families and individuals in Cameron and Willacy counties of health services availability, accessibility, and eligibility by providing educational outreach services. Educates families and individuals of preventative health care to promote a healthier community.
Coordinates events and fundraising activities for SCF. Provides referrals and patient registration follow-up increasing the number of patients we serve. Provide Population Management support within the clinic to assure patient retention, good health outcomes and a positive patient experience. Participates and manages grants associated with Outreach Department as requested, to assure criteria is met.
Using a variety of methods (face-to-face, telephone, home visits, hospital visits, community events, clinic lobbies, social media and patient mail outs) to provide essential support to clinical services as needed. Educates families and individuals in Cameron and Willacy counties of health services availability, accessibility and eligibility by providing educational outreach services.
Educates families and individuals of preventative health care to promote a healthier community. Conducts community group presentations, and other community activities to promote information. Works with families and individuals to increase access to health and social services.
Coordinates and/or participates in community health fairs to promote a healthy lifestyle. Develops and distributes informational printed materials such as posters, flyers, brochures and other promotional items to promote the goal of the program by working with the Outreach Director and the Planning Director.
Develops collaborative community relationships with local institutions such as employers, faith based organizations, schools and other community based social service agencies to promote the goals of the program.
Coordinates special events and fundraising activities for Su Clinica. Will provide communication to employees, co-workers, supervisor, maintenance staff, clinic supervisors, and Human Resources accounting on all special events which will impact these departments.
Supports Community Health Center Advocacy Campaigns. Refer clients to Outreach CAC's for application assistance on Marketplace, CHIP, and Medicaid.
Maintain Breast Feeding Peer Counselor status. Will work with Women's Health and Pediatric Departments to provide support to new mothers who may experience problems breastfeeding.
Prepares monthly report on outcome measures of activities, number of families encountered, number of patients served. Reports will be detailed, accurate and timely. Reports will be based on SCF Strategic Plan, SCF Business Plan and the Community Outreach Department goals and objectives. Works with grants under Community Outreach Department to assure compliance and meet criteria as assigned.
Responsible for own safety and the safety of others. Will comply with all infection control requirements of the clinic at all times. Approaches patient care as a team involving clinical and non-clinical staff (eg. Physicians, mid-levels, educators, appointment staff) to achieve patient care objectives.
High School Diploma or GED graduate with a minimum of two years experience in outreach activities. Bilingual (English/Spanish) required.
