One Health Behavioral Health Peer Support Specialist - Sacred Families in Harlem, Montana

POSITION SUMMARY: Functioning as a core member of the Sacred Families outreach team in the primary care clinic, the Behavioral Health Peer Support Specialist (PSS) is responsible for providing and coordinating warm handoffs and mentoring to patients or potential patient families who are expecting or have children up to age two (2), who are impacted by SUD (substance use disorder) and/or mental health challenges and in need of behavioral health services and support. The PSS also provides experience-based input to guide the implementation of the strategic plan for recruiting participating families to the Sacred Families Program according to the community’s unique needs. Clinical supervision of the PSS is provided by a licensed Behavioral Health Provider.

COMPENSATION: $18.31-18.90/hr DOE

EMPLOYMENT TYPE: Part Time - Full Time, 20-40 hrs/week

Specialist Duties and Responsibilities

  • Maintains functionality of organization and/or department by managing tasks and information to optimize processes.

    Position Specific Duties and Responsibilities

  • Maintains stable recovery and necessary care/treatment of behavioral health challenges and is willing to self-identify publicly as being in recovery from a mental health, substance use, or addiction diagnosis.

  • Obtains/maintains state certification for Peer Supporters.

  • Understands the concept of recovery as it relates to behavioral health; peer support/recovery coaching; wellness and recovery planning.

  • Facilitates patient engagement and follow-up care.

  • Documents in-person, video and telephone encounters in electronic health record and stays current on all necessary documentation.

  • Facilitates referrals for clinically indicated services outside the primary care clinic (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care).

  • Participates in multi-disciplinary, team-based care of patients/clients.

  • Participates in training, reporting, and other requirements of various grant supported programs implemented by One Health and partners.

  • Teaches a minimum of one section of the Fatherhood is Sacred/Motherhood is Sacred and/or Circle of Security Parenting classes per year.

  • Facilitates or cofacilitates birth education classes in the community as needed.

  • Facilitates trauma-centered education and recovery support groups as needed.

  • Facilitates recovery fitness groups for clients as needed/appropriate.

  • Offers Narcan training as often as needed for community groups and citizens.

  • Adheres to all One Health Policies and Procedures.

  • Participates in performance optimization and risk management activities as required.

  • Cooperates and abides by all elements of One Health’s Compliance Program.

  • Exemplifies a patient-centered care approach to each patient along each stage of the patient journey, while working as a key patient centered team member by communicating individual patient care needs to support the needs of patients served.

  • Provides exceptional customer service and models the mission of One Health when interacting with internal and external stakeholders promoting a positive image for the health center.

  • Performs other duties as assigned or required with or without prior notification.

    MINIMUM JOB QUALIFICATIONS:

    Education/Certificates/Licenses:

    Required

  • High School Diploma or GED

  • State certification for Peer Supporters (required to complete Peer Support 101 and begin certification filing process within 30 days of hire)

  • Master Narcan Trainer Certification (must be obtained within 60 days of hire)

  • Phoenix Multisport Trauma Informed Group Facilitation Certification (must be obtained within 90 days of hire)

  • BLS Certification within 30 days of hire

  • Valid driver’s license

  • Certified Trainer for Fatherhood is Sacred/Motherhood is Sacred

    Experience:

    Required

  • Lived experience with SUD and/or mental health challenges

  • Minimum two (2) years in successful recovery

  • Competent computer skills, including Microsoft Office
