DEFINITION:

Provides vision, strategy, and leadership in the planning, development, implementation, and management of prevention programs that enhance community health and well-being while fostering stakeholder and consumer relationships. Evaluates performance data to measure productivity and achieve goals. Establishes and enforces program policies, goals, objectives, and procedures.

SUPERVISION.

Position is supervised by the outpatient supervisor.

DUTIES AND RESPONSIBILITIES:

  • Actively participates as an integral part of the CCBHC multidisciplinary team. Attends and participates in agency and department/team meetings. Serves as an active member of the agency/community committees as directed and communicates engagement with team.
  • Enhances collaboration and engages agency and community providers in coordination of care including but not limited to discharge planning and connection to/from care with hospitals or other providers to NorthCare.
  • Identifies new partners and engages schools, hospitals, employers, and other stakeholders to conduct community outreach. Attends awareness events and distributes resources such as prevention, individual/family-centered, and recovery-oriented resources including but not limited to mental health first aid, suicide prevention, substance use prevention, and trauma informed communities aimed at target population.
  • Incorporates technology into the delivery of outreach activities and messages, including but not limited to, webinars, social networking, online web content, or other forums of information delivery.
  • Supports consumer and stakeholder survey completion, identifies program strengths and opportunities to improve, provides feedback to multidisciplinary teams based on survey results.
  • Performs basic accountability tasks, include compiling and maintaining all necessary records, assisting administrative staff in collection of information for program effectiveness, and ensuring all required documentation is submitted according to external and internal deadlines for reporting purposes.
  • Promotes a professional and organized culture within NorthCare and with all partnering agencies and stakeholders.
  • Perform other related duties as required and/or assigned by NorthCare Edmond Leadership Team and/or executive staff.
