GREENFIELD - SEE WEBSITE FOR FULL JOB DESCRIPTION - Clinical Services Nurse Manager The Client Services Nurse Manager is responsible for the administrative and clinical oversight of programs that complete assessments and redeterminations for individuals seeking a variety of community-based programs as well as eligibility for skilled nursing facilities. The Client Services Nurse Manager directly supervises the Home Care nurses and serves as a resource for all Client Services programs, including Adult Family Care (AFC) and Personal Care Attendant (PCA). The Client Services Nurse Manager participates as an active member of the interdisciplinary case management and coordinates nursing teams across the agency. QUALIFICATIONS: Registered Nurse with valid, current Massachusetts RN license. BSN from an accredited nursing school and three years clinical experience in a community setting or long term care setting and at least one year of supervisory, administrative or managerial experience. Organizational skills: Ability to manage time effectively, prioritize tasks, set goals, and develop systems for achieving those goals. Able to attend to multiple responsibilities simultaneously and handle complex projects. Critical Thinking: Highly adept in organizing complex planning processes involving diverse groups of stakeholders. Ability to lead people through problem-solving processes. Quality Improvement Focus : Knowledge of key drivers of quality assurance, quality improvement and quality enhancement initiatives. Communication & Interpersonal Relationships: Professional verbal and written communication skills. Good listening and engagement skills. Ability to work well across disciplines and with diverse teams. Coaching & Staff Development: Values each staff member and their contributions. Provides feedback on performance and assists staff to develop to their fullest. Leadership Skills: Influences others through positive role modeling. Assist with training others. Results Driven: Self-motivated and proven ability to meet short-term and long-term goals Computer & Technical Proficiency: Intermediate to advanced practical application of MS Word, Excel, and Google suite skills. Competence in virtual platforms such as Zoom/Google Meet Advanced ability to manage electronic databases and other electronic platforms. . DUTIES AND RESPONSIBILITIES: Provides oversight of the Home Care Nursing Program which completes clinical assessment and eligibility (CAE) determinations for both Nursing Facility and Community Based programs. . Works closely with the Program Directors and Supervisors of the PCA and AFC program to provide clinical and administrative oversight of nursing functions. Provides consultation to all Client Services departments regarding clinical issues. Participates in service coordination and case conferences with clients, their families, LifePath staff, community agencies, nursing facilities and other health/community professionals as needed including multidisciplinary team meetings for Housing and AFC Conducts quality assurance/quality improvement (QA/QI) activities for the Home Care Nursing function and assists QA/QI for nursing functions within the PCA and AFC programs. Work closely with other Client Services Program Directors on policies and procedures and operations which involve the nursing function in departmental programs. Provide quarterly orientation and refreshers to new and existing staff regarding the role of the Home Care nursing team. Supervise Home Care RNs. Participate in the interviewing and hiring process. Routinely evaluate RN caseloads and trends
