PLS provides residential services for ambulatory adults who have a diagnosis of intellectual disability or mental illness that require minimal personal cares. We offer various programs such as Community Based Residential Facility, certified 2 bed, Adult Family Home, apartment style, and supportive home care. We have 24-hour staffing, an awake third shift and multiple locations in Appleton, Kimberly, Oshkosh, Neenah and Green Bay! The work is interesting, challenging and rewarding. Job duties include, but are not limited to:

  • Engage clients in a variety of activities that build self-esteem and expands client’s quality of life

  • Implement client’s Individualized Service Plan

  • Chart daily progress notes and flow sheets

  • Monitor medication to clients

  • Assist the clients in learning daily and independent living skills

  • Accompany your client to a doctor appointment

  • Provide transportation for errands or activities

  • Plan and prepare meals

  • Light housekeeping—dusting, vacuuming, laundry, etc.

  • Encourage and facilitate social activities and exercise

  • Provide conversation and a listening ear

  • At times provide additional types of care such as bathing and incontinence care.


  • Must be 18 years or older

  • Must have a high school diploma or GED/HSED equivalent

  • Basic knowledge of Microsoft Word or Excel

  • Ability to maintain self-composure under stressful conditions. (Clients may exhibit aggressive, challenging, or offensive behavior.)

  • Ability to fully communicate both written and verbally

  • Proof of a valid Wisconsin driver's license w/ clean driving record

  • Successful completion of pre-employment physical, TB test, caregiver background check and applicable trainings

  • Available to work every other weekend & holidays

Our programs are staffed 24 hours a day / 7 days a week / 365 days a year - being available to work every other weekend and some holidays is a requirement of the position.
