Corewell Health Physician- Embedded Cardiologist in Primary Health in Grand Rapids, Michigan
Corewell Health West's Primary Health department is seeking to embed a cardiologist into the primary care team for greater coordination and integration between primary and secondary care in a value-based care model. This model is designed to demonstrate a meaningful impact on patient outcomes for defined populations of patients while helping to improve access to cardiology, adherence to clinical guidelines, key outcomes for CVD and to decrease unnecessary referrals to specialty care.
Essential Functions
Assesses and monitors the medical needs of patients/significant others.
Provides quality medical services to patients in a cost effective, appropriate manner and ensures coverage of case load at all times.
Provides input into the development of policies and procedures as they related to medical practice and quality patient care.
Provides oversight of Physician Assistants and Nurse Practitioners and others providing delegated services to patients.
Evaluates patients'/significant others'/staff educational needs and develops and implements appropriate teaching strategies or delegates to appropriate resources.
Documents in a manner consistent with regulatory standards.
Communicates with other members of the health care team in order to ensure continuity of care and coordination of services. Acts as a patient/family advocate.
Participates in ongoing process improvement activities through the development/application of clinical paths, process improvement and other related activities.Actively works with Spectrum Health to meet financial and patient satisfaction goals.
Qualifications
Required Bachelor's Degree
LIC-Physician (MD) - STATE_MI State of Michigan Upon Hire required Or
LIC-Osteopathic Physician (DO) - STATE_MI State of Michigan Upon Hire required
CRT-DEA Registration - STATE_MI State of Michigan Upon Hire required
CRT-Basic Life Support (BLS) - AHA American Heart Association 120 Days required Or
CRT-Basic Life Support (BLS) - ARC American Red Cross 120 Days required
CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association If required by specialty or stress test performed 120 Days required
