This position has a residency waiver and will be accepting applications from residents and non-residents of Colorado. This announcement is open to potential candidates who are legal residents of the United States. All legal residents of the United States are eligible to apply and compete. This position will receive a short-staffing temporary pay differential of 8% from July 1st 2024 - June 30th 2025.

About the Grand Junction Regional Center:

This position resides at the Grand Junction Regional Center. This regional center exists to manage the day-to-day operations and service delivery for Grand Junction Regional Center (GJRC), licensed as an Intermediate Care Facility for Individuals with Intellectual Disabilities as well as a provider of Home and Community Based Services (Waiver services).GJRC provides 24/7 direct care for approximately 80 residents. GJRC's primary goal is to provide the necessary supports and services to residents who cannot be safely served in community based programs due to complex medical and/or behavioral needs.

Description of Job

The Clinical Specialist is responsible for overseeing all medications for individuals residing under the Intermediate Care Facility (ICF) license and all over-the-counter medications for individuals residing under the HCBS Waiver license at the Grand Junction Regional Center (GJRC). This position also provides oversight for all ordering and inventory of over the counter medications and manages medication error resolution, tracking, documentation and reporting.

Additionally, as a Clinical Specialist you can expect to:

  • Responsible for the accuracy and ensures quality control of the of the individual MARs located in the electronic medical records program. Ensures that MARs are updated accurately for all orders that are entered into the MAR and communicates with team nurses if changes are needed to the MAR

  • Utilizing the electronic records management program; tracks, manages, completes follow up, documents in electronic health record and reports on medication errors; verifies that nurses notified parent/guardian and providers are notified. Ensures protocols for medication errors are followed. Assists nursing team with medication administration competencies quarterly and annual checks

  • Determines the need for a physical and/or occupational therapy care plan by collaborating with physicians, therapists, team nurses, direct care staff or the individual. Attends therapy appointments. Receives specific instructions from physicians and therapists for therapy care programs or seeks out instruction when the functions are beyond basic restorative scope. Helps to facilitate continuity of care when a person moves from one living area to another or is transported to or returns from the hospital

  • Responsible for oversight of the psychiatric appointment orders and resulting medications changes, including: ensuring documentation is updated by the team nurses, verifying documentation regarding all updates and changes are noted in the electronic record, including verifying guardian notification has been completed regarding prescription medication changes

  • Track informed consents are completed annually and will prepare informed consents for all psychotropic medications prescribed to individuals at GJRC. Will also complete ongoing updates for all informed consents

  • Collaborates with the Director of Nursing Services in planning, directing, coordinating, and supervising the activities of the nursing night/weekend shifts. Ensures assignments are completed and assessments are done in a timely manner and documentation is completed according to agency procedures

  • This position works to develop efficiencies in the night/weekend shift nursing staff and makes recommendations to the Director of Nursing for quality improvement. Directly supervises assigned unit nurses that includes signature authority that impact pay as well as doing the Performance Management plans and appraisals. This position has corrective action authority for performance problems. Ensures biweekly 1:1 meetings are planned and completed with all weekend and night nurses

  • Under the direction of the Director of Nursing, this position is the Health Risk Screening Tool (HRST) liaison or Gatekeeper for the GJRC Nursing division. The HRST is a simple 22 item scale designed to find out who is at the greatest risk of illness and developed to detect health destabilization in vulnerable populations.

Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

Minimum Qualifications:

Education/Licensure/Certification/Experience:

  • Current, valid licensure as a Registered Nurse from the Colorado Board of Nursing or any state participating in the Enhanced Nurse Licensure Compact (eNLC)

AND

  • Three (3) years of professional experience in an occupational field related to the work assigned to the position

Substitutions:

  • Appropriate education will substitute for up to two (2) years of the required experience on a year-for-year basis

Conditions of Employment:

  • CDHS employees (all Direct contact with vulnerable persons): CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference

Checks, Professional License verification (licensure requirements), Drug Screen, PREA (Division of Youth Services), Trails check (direct contact with children), CAPS (direct contact with adults – Mental Health Institutes, Regional Centers, Veterans Community Living Centers). A Tuberculosis Test (TB) will be required upon hire

  • Must possess a valid, unrestricted U.S. Driver's License

  • Must have current AHA BLS CPR certification

  • Must be 18 years or older

  • Shift Work – 24/7 nursing coverage, duties may require work on multiple shifts including day, nights, weekends, holidays

  • Travel required - Must provide services to persons and multiple living and program sites and may need to transport residents in State vehicles. If a State vehicle is not available, must have their own vehicle for purposes other than transporting residents.

  • Former State employees who were disciplinarily terminated or resigned in lieu of termination must (1) disclose that information on the application (2) provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position, (3) provide the employee number from the applicant’s prior State employment. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination and failure to provide this information will disqualify the applicant from future State employment with CDHS.
