This position has a residency waiver and will be accepting applications from residents and non-residents of Colorado. This announcement is open to potential candidates who are legal residents of the United States. All legal residents of the United States are eligible to apply and compete. This position will receive a short-staffing temporary pay differential of 3.17% from July 1st 2024 - June 30th 2025

About Grand Junction Regional Center (GJRC)

The Grand Junction Regional Center exists to provide support to individuals with intellectual and developmental disabilities (IDD), medical and psychiatric needs. These individuals live in 11 group homes located in the Grand Junction community. The Nursing Department provides comprehensive nursing care to individuals with IDD served under two distinct licenses, Intermediate Care Facility (ICF) and Home and Community Based Services (HCBS) Waiver group homes in the community setting.

Description of Job

About the Position:

This work unit exists to provide quality health care, personal hygiene maintenance, and an active learning atmosphere in a safe and healthy environment for individuals with intellectual and developmental disabilities. This work unit provides 24-hour treatment services that comply with State and Federal regulations. This position will act as a Mentor to all designated staff that provide direct care services to our individuals to give support and services to people in the areas of health, safety, activities of daily living and also providing opportunities for people to achieve their desired personal outcomes as described in their Individual Plans. This position will work in direct collaboration with the Staff Development Department training team to ensure proper training, documentation, and support of mentors.

Duties in this position include:

  • Provide mentoring to employees that are assigned to this program; demonstrate related competencies and observe return demonstration, provide on-the-spot training, evaluate and document that each competency was demonstrated proficiently

  • Provide follow-up communication with assigned employee(s) at regular intervals, as outlined by program

  • Provide training and demonstration in methods and techniques that are approved and supported for individuals (or teams) that struggle to be successful

  • Train newly assigned staff in work duties and schedules, according to pre-established activity plans/schedules and daily individual needs

  • Report any issues/concerns; monitor area/individuals to ensure safe environment

  • Provide care such as bathing, eating, personal hygiene, and instruction on daily living activities for persons receiving services

  • Administer all treatments, medications, and dietary orders as prescribed, including preparing meals; determine and provide the best possible environment to meet individual needs, safety, and personal outcomes

  • Respond to emergencies and assist medical staff with medical support and services

  • Document and/or communicate per protocols/procedures all pertinent information regarding residents for whom this position is responsible for at any given time

  • Transport, or coordinate transportation for, individuals receiving services to/from appointments, scheduled events, and emergency transports

  • Work other locations and/or shifts as needed to address agency resource needs or to provide support to staff (or teams) that are struggling; may be pulled to work multiple areas within a shift and will drive to each location

  • Other duties as assigned

Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

Minimum Qualifications:

Education/Licensure/Certification/Experience:

  • High school diploma or General Education Diploma (GED)

AND

  • One (1) year of experience in an occupational field related to the work assigned to the position

Substitution:

Additional appropriate education will substitute for the required experience on a year-for-year basis

Special Qualifications:

  • Must complete the mentor program training (one week long) within one year of employment

    Preferred Qualifications:

  • Experience mentoring and coaching

  • Must possess a valid LPT license or be QMAP trained

  • Must be annually certified in physical intervention techniques

Conditions of Employment:

  • CDHS employees (all direct contact with vulnerable persons): CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference

Checks, Professional License verification (licensure requirements), Drug Screen, PREA (Division of Youth Services), Trails check (direct contact with children), CAPS (direct contact with adults – Mental Health Institutes, Regional Centers, Veterans Community Living Centers)

  • Shift Work – GJRC is a 24/7 facility. Person in this position must be available to work any shift, any day, and/or any area to meet Agency needs.

  • Must have valid unrestricted U.S. Driver License.

  • Travel - Position must travel to remote work sites and must utilize a State vehicle, if available. If not available, must utilize personal vehicle and submit for mileage reimbursement

  • Pre-employment Physical/Tests: Must demonstrate the ability to lift 50 lbs, bend, squat, etc. and perform physical interventions proficiently (pre-employment and continuously throughout employment). TB test required.

  • Former State employees who were disciplinarily terminated or resigned in lieu of termination must disclose the information on the application and provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination will disqualify the applicant from future State employment with CDHS.
