POSITION OVERVIEW:

This position is responsible for providing options counseling, resource and referral and case management services for the Hilltop s Aging and Disability Resources for Colorado (ADRC), and assists clients with specific applications. Develops partnerships with community agencies, organizations, businesses and individuals to grow the reach of ADRC. This position will also serve as the referral entry point for the Colorado Transitions Coordination program.

This position has a full-time schedule of Monday-Friday from 8a-5p.

This position has a starting wage range of $20.00-$21.00 per hour.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Responsible for answering the ADRC main line from Monday through Friday during which they assess callers needs and provide appropriate referrals to community agencies. Meets with individuals with physical, mental, and/or cognitive disabilities and provides person-centered approaches to help individuals gain an understanding of the benefits and limitations of long-term services and support options; connects individuals to services and acts as a liaison with service agencies. Maintains current and in-depth knowledge of long-term care services available in the community. Assists with filling out applications for specific, needed programs. Follows up with clients as necessary to ensure service connections and delivery. Maintains clear, accurate and timely records in the State s data base (SUDS), including log notes, assessments, care plans, and other relevant documents. Manages other pertinent documents including release of information forms, status reports, etc. Provides short-term case management for clients accessing ADRC voucher services and long-term case management for up to 10 cases of the ADRC s Home Connections program Fiscally responsible for services approved and provided under the voucher services and Home Connections program which includes, making credit card purchases, completing check requests, completing appropriate coding and financial steps required for providers to be paid and communicating with providers about approved services. Assists with the managing and supervision of ADRC s Supporting Our Seniors (SOS) volunteers, including scheduling, maintaining proper documentation, addressing volunteer issues and providing recognition. Colorado Transitions Coordination Program - ADRC is the initial point of entry for this program and provides Options Counseling and information to nursing home residents interested in enrolling in this program. Data collection and on-going training and community collaboration is also required. Maintains reports for the ADRC program and appropriate funding agencies. This includes updating tracking sheets for the ADRC monthly reports, mileage, training hours, community outreach, Medicaid applications, monthly surveys, etc Collaborates with community partners to ensure the ongoing success of the ADRC. Seeks out resources to meet the needs of clients. Attends various community meetings and participates on community committees and coalitions. Provides presen ations to various community agencies and groups. Helps to facilitate various evidenced-based programming including participating in training and coordinating classes with various local agencies and participants.
