Description - External The Pharmacist, Clinical Specialist Oncology (PCSO) is responsible for: providing quality patient care in relation to the patient’s oncologic/hematologic diagnosis, prescribed treatment, age group, and other identified needs; providing comprehensive pharmaceutical care to oncology/Hematology patients to assure safe and effective drug therapy. The PCSO coordinates the drug therapy process for the oncology/Hematology patient population and the role may include any or all of the following activities: drug selection, drug information, dosing, monitoring, outcomes management, and patient education/counseling. This position requires independent decision-making, clinical judgment, and in-depth knowledge of oncology/hematology therapeutics. Board Certified Oncology Pharmacist (BCOP) credential or 10 years of active oncology experience preferred. Qualifications - External Education & Experience * Graduation from a pharmacy program accredited by the Accreditation Council for Pharmacy Education (ACPE) * Minimum of 3-years’ experience with at least 50% in oncology; PGY1 residency plus two (2) years of practice experience including 50% or more of that * Time spent in oncology or completion of a specialty (PGY2) residencyin oncology pharmacy plus one (1) additional year of practice with at least 50% of * Time spent in oncology pharmacy activities. * PharmD Preferred Licenses, Certifications, Registrations * Current, active license to practice pharmacy in New York State; or eligible for reciprocate licensure from another state. * Pharmacist Board Certification Oncology, required within two years of hire or required within 2 years of eligibility to sit for certification exam. Salary Range The expected base rate for this Glens Falls, New York, United States-based position is $48.99 to $73.48 per hour.
