Finger Lakes Community Health (FLCH) was founded in 1989 with an original mission of serving the region's agricultural workers by linking individuals to area resources and providing essential health services. In 2009 the organization expanded to become a Federally Qualified Health Center (FQHC), serving patients of all incomes, ethnicities and walks of life. Over the last decade, FLCH has grown to provide medical, dental, and behavioral health services to over 28,000 patients at eight health centers. Job Summary:
The role of a HHUNY Care Manager is to be the key point of contact in managing health services. The Care Manager will be the center point of the communication to ensure all providers are working together with the patient in mind. The Care Manager will conduct comprehensive assessments in order to obtain the most accurate information to build off of the identified needs and risk factors.
Duties/Responsibilities:
Responsible for, but not limited to comprehensive assessments, outreach and engagement, service and treatment linkage, and coordination
Develops client care plan, supports adherence, and maintains client and care team communication to ensure adherence
Comprehensive client-centered care planning development and management with the individual to assist in the integration of medical and behavioral health services, and social health services
Coordination of care with community service providers that are part of the client care team
Assessment of psychosocial supports and linkages with medical, dental, and behavioral health care providers, as well as, educational, employment, transportation community resources
Build and use effective communications strategies among peers, medical staff, addiction and mental health providers, and other community agencies through the use of electronic assisted devices including Telehealth and other interactive technology
Operates as the point of care for Prevention, Wellness, Medical Treatment, Behavioral Health, Transition Care, Social Needs
Help improve, measure, monitor, and sustain quality outcomes that focus on clinical indicators/performance measures, patient satisfaction, and plan adherence
On-Call duties as assigned for nights and weekends
Required Skills/Abilities:
Possesses excellent interpersonal, communications, public speaking, and presentation skills
Possesses basic computer skills, including Microsoft Office and the ability to learn new software
Possesses strong organizational skills
Possess the ability to prioritize
Possesses the ability to work effectively with culturally diverse populations
Education and Experience:
High School or Equivalency Diploma
Minimum of 2 years of experience in a similar role
Possesses New York State driver’s license
Current New York State automobile insurance in compliance with FLCH underwriting agreement
Physical Requirements:
Must be able to hear and communicate with clients and staff via phone, web media, and in person.
Must be able to lift up to 30 lbs
Must be able to sit for extended periods of time
